剖宫产瘢痕妊娠行宫腹腔镜联合治疗的分析及临床评价

摘要
目的探讨剖宫产瘢痕妊娠行宫腹腔镜联合治疗的观察及临床评价。方法 2015年4月—2018年9月采集68例剖宫产瘢痕妊娠孕妇随机数字表分组。对照组进行常规治疗,观察组进行宫腹腔镜联合治疗。分析效果。结果观察组手术效果34例(100.00%)更高,对照组是27例(79.41%)(χ~2=7.844,P<0.05)。观察组手术实施时间、术后阴道出血时间、β-hCG恢复正常时间(51.02±10.12)min、(3.01±2.61)d、(5.21±1.21)周优于对照组(t=7.333,8.042,9.322,P<0.05)。对照组手术实施时间、术后阴道出血时间、β-h CG恢复正常时间分别是(62.14±13.24)min、(7.15±3.57)d、(10.62±2.59)周。观察组并发症低于对照组(χ~2=6.233,P<0.05),其中,对照组有3例感染,2例出血,2例发热和1例腹痛,总发生率是9例(26.47%)。观察组有1例感染,1例出血,2例发热,总发生率是3例(8.82%)。施术后观察组β-hCG水平(14.24±0.11)IU/L低于对照组(78.91±1.34)IU/L(t=7.022,P<0.05)。差异有统计学意义(P<0.05)。结论剖宫产瘢痕妊娠孕妇行宫腹腔镜联合治疗效果确切。
Objective To investigate the observation and clinical evaluation of laparoscopic combined treatment for cesarean section scar pregnancy. Methods A randomized digital table of 68 pregnant women with cesarean section scar pregnancy was enrolled. The control group received routine treatment and the observation group underwent combined laparoscopic treatment. Analyze the effect. Results The surgical outcome of the observation group was higher 34 cases(100.00%), and the control group was 27 cases(79.41%)(χ~2=7.844, P<0.05). The operation time of the observation group,the postoperative vaginal bleeding time, the normal time of β-hCG recovery were(51.02±10.12)minutes,(3.01±2.61)d,(5.21±1.21)weeks, which were better than the control group(t=7.333, 8.042, 9.322, P<0.05), respectively. The operation time of the control group, the postoperative vaginal bleeding time, and the normal time of β-hCG recovery were(62.14±13.24)minutes,(7.15±3.57)d,(10.62±2.59)weeks, respectively. The complication of the observation group was lower than that of the control group(χ~2=6.233, P<0.05). Among them, there were 3 cases of infection, 2 cases of bleeding, 2 cases of fever and 1 case of abdominal pain in the control group. The total incidence was 9 cases(26.47%). There was 1 infection in the observation group, 1 case of hemorrhage, and 2 cases of fever. The total incidence was 3 cases(8.82%). The level of β-hCG in the observation group was(14.24 ±0.11)IU/L lower than that in the control group(78.91±1.34) IU/L(t=7.022, P<0.05), the difference was statistically significant(P<0.05). Conclusion The laparoscopic combined treatment of cesarean section scar pregnancy is effective.
