高敏C反应蛋白与载脂蛋白A-1、甘油三酯联合检测在心血管疾病中的临床价值

摘要
目的分析在心血管疾病中联合应用hs-CRP、apo A-1及TRi检测的临床价值。方法选取我院2016年5月~2018年6月收治的患有心血管疾病患者90例进行研究,根据疾病的不同,将其分为急性心肌梗死组30例、稳定型心绞痛组30例、不稳定型心绞痛组30例,并选取同期健康体检结果为正常的患者29例,将其作为对照组。收集所有患者的临床资料,并对其hs-CRP、apo A-1及TRi进行检测。观察并比较各组患者的hs-CRP、apo A-1、TRi水平,并分析hs-CRP、apo A-1、TRi联合检测对心血管疾病诊断的敏感性、特异性。结果稳定型心绞痛组、不稳定型心绞痛组、急性心肌梗死组的hs-CRP、TRi水平显著高于对照组;稳定型心绞痛组、急性心肌梗死组的hs-CRP显著高于稳定型心绞痛组;稳定型心绞痛组、不稳定型心绞痛组、急性心肌梗死组的apo A-1水平显著低于对照组;hs-CRP、apo A-1、Tri联合检测对稳定性心绞痛组、不稳定型心绞痛组、急性心肌梗死组的敏感性、特异性显著高于hs-CRP、apo A-1、Tri单独检测的敏感性、特异性,差异有统计学意义(t=1.225,P <0.05)。结论心血管疾病患者的高敏C反应蛋白、甘油三酯水平显著升高,而载脂蛋白A-1显著降低,联合检测hs-CRP、apo A-1、TRi对心血管诊断有一定的临床价值。
        Objective To analyze the clinical value of combined test of high-sensitivity C-reactive protein(hsCRP),apolipoprotein A-1(apo A-1),triglyceride(TRi) in cardiovascular diseases. Methods Ninety patients with cardiovascular diseases in hospital from May 2016 to June 2018 were selected and divided into acute myocardial infarction group(30 cases),stable angina pectoris group(30 cases) and unstable angina pectoris group(30 cases),and 29 patients with normal physical examination results were selected as control group.The clinical data of all patients were collected, and hs-CRP,apo A-1 and TRi were detected.The levels of hs-CRP,apoA-1 and TRi in each group were observed and compared,and the sensitivity and specificity of combined test of hs-CRP,apoA-1 and TRi in the diagnosis of cardiovascular diseases were analyzed. Results The levels of hs-CRP and TRi in stable angina pectoris group,unstable angina pectoris group and acute myocardial infarction group were significantly higher than those in control group.The levels of hs-CRP in stable angina pectoris group and acute myocardial infarction group were significantly higher than that in stable angina pectoris group.The levels of apo A-1 in stable angina pectoris group,unstable angina pectoris group and acute myocardial infarction group were significantly higher than that in stable angina pectoris group.The sensitivity and specificity of combined test of hs-CRP,apo A-1 and Tri for stable angina pectoris,unstable angina pectoris and acute myocardial infarction were significantly higher than those of hs-CRP,apo A-1 and Tri alone(t=1.225,P=0.01). Conclusion The levels of hs-CRP and TRi in patients with cardiovascular diseases are significantly increased,while apo A-1 is significantly decreased.Combined testof hsCRP,apo A-1 and TRi has certain clinical value in cardiovascular diagnosis.
