427例痛性糖尿病神经病变患者中医证候分布特征分析

目的:研究痛性糖尿病周围神经病变的中医证候特点及其影响因素。方法:通过对427例确诊为痛性糖尿病周围神经病变的患者中医证候进行观察,了解各证候患者的分布特征,分析其与患者的性别、年龄、病程及部分生化指标的关系,利用统计学方法对数据进行分析。结果:痛性糖尿病周围神经病变患者中以阴虚血瘀证(23.88%)、痰凝血瘀证(31.85%)最为多见,随着年龄的增长寒凝血瘀型和肝肾亏虚型的比例有所增加,而阴虚血瘀型比例减少,随着病程的延长,阴虚血瘀证组比例逐渐下降,而肝肾亏虚证组的比例呈升高趋势,痛性糖尿病周围神经病变不同的证候分型与病程和年龄均有显著相关性。但不同证型的痛性糖尿病周围神经病变患者与空腹血糖(Glucose,GLU)、餐后2小时血糖(2 hours Postprandial Blood Glucose,2 h PBG)、糖化血红蛋白(Hb A1c)无显著相关性。结论:探讨痛性糖尿病神经病变中医证候特点,可以为痛性糖尿病周围神经病变的中医辨证提供临床依据。
Objective: To study the characteristics of TCM syndromes and its influencing factors in painful diabetic peripheral neuropathy. Methods: The distribution characteristics of TCM in 427 patients with painful diabetic peripheral neuropathy were observed; the relationship between TCM syndrome type and gender, age, disease course and some biochemical indicators was analyzed by statistical methods. Results: Among all patients, the Yinxu Xueyu syndrome(阴虚血瘀证)(23.88%) and the Tanning Xueyu syndrome(痰凝血瘀证)(31.85%) were the most common. As the age increases, the proportion of the Hanning Xueyu type(寒凝血瘀型) and the Ganshen Kuixu type(肝肾亏虚型) were increased, and the proportion of the Yinxu Xueyu type(阴虚血瘀型) was decreased. As the disease progresses, the proportion of the Yinxu Xueyu syndrome(阴虚血瘀证) was decreased, while the proportion of the Ganshen Kuixu syndrome(肝肾亏虚证) was increased. There was a significant correlation between syndrome type and disease duration and age; there was no significant correlation with GLU, 2 h PBG and HbA1 c. Conclusion: The characteristics of TCM syndromes of painful diabetic peripheral neuropathy were analyzed, in order to provide clinical treatment basis of TCM syndrome differentiation.
