TCT联合高危型HPV检测诊断宫颈癌的研究进展

摘要
液基薄层细胞学检测(TCT)取材及制片均比较简单,能够进行系统程序化的处理,对宫颈上皮内异样细胞的检出率明显高于传统的巴氏涂片法,同时能够弥补传统涂片的不足与缺陷,提高标本的可信度,降低假阴性率。人乳头瘤病毒(HPV)在宫颈癌及癌前病变发生、发展中起到关键性作用。高危型HPV与宫颈癌的发生密切相关,因此高危型HPV DNA检测是筛查宫颈癌及癌前病变最直接、有效的方法,能够减低TCT检测的假阴性率。并且高危HPV检测的阳性率随着TCT检测分级的增高而上升,对TCT检测中的不能明确诊断意义的不典型鳞状上皮细胞(ASCUS)及以上的病变行高危HPV检测能够分流检测结果,更精准地筛选高风险人群,提高早期筛查的针对性。TCT联合高危型HPV检测能够提高宫颈癌的筛查效率,提高检查准确性和特异性。
        Liquid thin-layer cytology test(TCT) materials and production are relatively simple, ability of system processing, stylized for detection of cervical intraepithelial strange cells was obviously higher than that of the traditional pap smear method, can make up for the inadequacy of traditional smear and defects at the same time, improve the credibility of specimens, reduce the false negative rate. Human papilloma virus(HPV) plays a key role in the occurrence and development of cervical cancer and precancerous lesions,high-risk type HPV is closely related to the occurrence of cervical cancer, so high-risk HPV DNA detection is the most direct and effective method to screen cervical cancer and precancerous lesions that can reduce the false negative rate of TCT detection. The positive rate of high-risk HPV test increased with the increase of TCT grade. High-risk HPV detection can shunt the detection results of atypical squamous epithelial cells(ASCUS) and lesions above which TCT can not be clearly diagnosed, and improve the pertinence of early screening. TCT combined with high-risk HPV test can improve screening efficiency and improve accuracy and specificity of cervical cancer.
