颞下颌关节假体置换术后口腔细节管理效果评估

摘要
目的:评估个体化颞下颌关节假体置换术后实施口腔细节管理的效果。方法:选取2010-02～2017-05在我科基于3D数字化技术定制个体化颞下颌关节假体进行单侧颞下颌关节置换的患者10例,在实施常规护理的基础上,随机分为对照组和改进组,每组5例。对照组使用常规棉球口腔擦拭,改进组实施细节管理措施,即注重患者口腔护理的方法细节处理。比较两组术后第2天、2周的菌斑指数以及术前、术后2周两组的牙龈指数,并评定两组术后3个月的咀嚼功能(包括咀嚼效率和咀嚼周期)。结果:持续冲洗2周后,改进组菌斑指数和牙龈指数显著低于对照组(P<0.01);改进组咀嚼效率较对照组明显升高(P<0.01);改进组咀嚼周期较对照组明显缩短(P<0.05)。结论:口腔护理细节管理对于个体化颞下颌关节置换术后患者口腔功能的恢复起到了重要作用。
Objective:To evaluate the effect of detail management on the postoperative nursing care for individualized temporomandibular joint(TMJ)prosthesis replacement.Methods:Ten cases with routine nursing undergoing individualized TMJ prosthesis replacement based on 3 D digital technology were selected from February 2010 to May 2017 in our hospital,and were randomly divided into two groups,5 cases in each group.The control group was received traditional methods of oral rinse,the improvement group was cleaned with orthodontic toothbrush and cotton swabs to improve the effects of oral rinse.The dental plaque index(PLI)of two groups at the second day and two weeks after operation,and the gingival index(GI)of two groups before operation and two weeks after operation were compared.The masticatory function(MF)include masticatory efficiency(ME)and masticatory cycle(MC)were measured before operation and three months after operation between the two groups.Results:PLI and GI of the improvement group were less than those of the control group(P <0.01).ME of the improvement group was higher than that of the control group(P <0.01).MC of the improvement group was shorter than that of the control group(P <0.05).Conclusion:The efficient postoperative nursing care play a important role in the postoperative oral function recovery of patients who underwent individualized TMJ prosthesis replacement.
