560份不合理用药医嘱分析

摘要
目的:了解解放军总医院第五医学中心(以下简称"我院")住院患者医嘱用药情况,为临床合理用药提供参考。方法:利用医院信息系统结合PASS合理用药审核软件,对2017年我院住院药房医嘱进行统计分析。结果:不合理用药医嘱共560份,其中,重复给药医嘱264份(占47.14%),用法与用量不合理医嘱254份(占45.36%),有配伍禁忌或相互作用医嘱20份(占3.57%);不合理用药医嘱主要涉及肝胆疾病用药(146份,占26.07%)、营养补充剂(104份,占18.57%)及消化系统用药(100份,占17.86%);不合理用药医嘱主要分布于感染科(70份,占12.50%)、放射治疗科(58份,占10.36%)及中西医结合科(55份,占9.82%)。结论:药师应参与药物临床应用,对医嘱进行审核干预,以提高合理用药水平。
OBJECTIVE: To investigate the drug application of inpatient medical orders in the Fifth Medical Center of PLA General Hospital(hereinafter referred to as "our hospital"), so as to provide references for clinical rational drug application. METHODS: Inpatient medical orders in or hospital in 2017 were collected and analyzed by using hospital information system and rational medication review software of PASS. METHODS: There were 560 irrational medical orders in total, of which 264(47.14%) were repeated administration, 254(45.36%) were irrational usage and dosage, 20(3.57%) were incompatibility or interaction; the irrational medical orders mainly involve hepatobiliary diseases(146, 26.07%), nutritional supplements(104, 18.57%) and digestive system medications(100, 17.86%); the irrational medical orders were mainly distributed in department of infectious disease(70, 12.50%), department of radiology(58, 10.36%) and department of integrated chinese and western medicine(55, 9.82%). CONCLUSIONS: Clinical pharmacists should participate in clinical medication, conduct review and intervene on medical orders to improve rational drug application.
