云南省5086例HIV感染产妇配偶HIV检测情况与影响因素分析

摘要
[目的]了解云南省艾滋病病毒(HIV)感染产妇配偶HIV检测情况及影响因素,为制定消除艾滋病母婴传播政策提供科学依据。[方法] 2012年1月—2016年6月,收集在云南省所有助产医疗机构内分娩的HIV感染产妇与配偶的相关信息,通过国家预防艾滋病、梅毒和乙肝母婴传播信息管理系统上报审核和汇总。使用描述性分析配偶的HIV检测情况,χ~2检验和多因素Logistic回归分析配偶HIV检测的影响因素。[结果]共有5086例HIV感染产妇配偶纳入此研究,配偶HIV检测率为81. 3%(4133例),在接受过检测的配偶中,检测结果阳性的占32. 7%(1664例),HIV阳性配偶报告的传播途径中异性性传播占77. 1%(1283例),注射吸毒的占12. 7%(212例),影响配偶HIV检测的主要因素为婚姻状况、配偶的民族、职业、文化程度和产妇是否在孕早期诊断HIV感染。[结论]云南省HIV感染产妇配偶HIV检测率和检出阳性率存在明显地区差异,可结合配偶HIV检测的影响因素,开展针对性的健康教育与促进咨询检测活动,提高HIV感染产妇配偶HIV检测率。
Objective To understand HIV detection status and influence factors of HIV infected pregnant women's spouses in Yunnan Province so as to provide scientific basis for formulating policy to eliminate HIV mother-to-child transmission( MTCT). Methods It collected relevant information on puerperal HIV-infected pregnant women in all deliver medical institutions and their spouses in Yunnan Province from Jan. 2012 to Jun. 2016. Reported,audited and summarized through National Surveillance System for Prevention of AIDS,Syphilis and Hepatitis B MTCT. Descriptively analyzed the spouses HIV detection status,Chi-square tests and multivariate logistic regression analysis were used to analyze influence factors. Results A total of 5086 HIV-infected pregnant women and their spouses were included in the study. The detection rate of the spouses was 81. 3%( 4133 spouses). In detected spouses,positivity of detection result was 32. 7%( 1664 cases). There were77. 1%( 1283 cases) heterosexual sexual transmission in HIV positive spouses report. Injection drug use accounted for 12. 7%( 212 cases).Main factors of affecting spouses HIV detection were marital status,nationality of spouses,occupation,education level and whether had HIV infection diagnosis in early pregnancy. Conclusions There is significant regional difference in HIV detection rate and positive detection rate among spouses of HIV-infected pregnant women in Yunnan Province. Health education and health promotion targeted on specific risk factors can be carried out to increase HIV detection rate among spouses of HIV-infected pregnant women,so as to improve HIV detection rate of HIV-infected pregnant women's spouses.
