角膜内皮移植术后患者头部固定枕的设计与应用

摘要
目的:设计一种经济实用的头部固定枕,以帮助角膜内皮移植术后患者的头部固定及维持术后长时间仰卧位。方法:选取2014年6月-2017年2月于武汉大学人民医院眼科中心行角膜内皮移植术的30例患者(30只眼)为研究对象,按入院顺序分成试验组(15例)和对照组(15例)。两组患者术后均保持长时间仰卧位,试验组加用头部固定枕,对照组不使用任何辅助工具。观察两组患者术后舒适度和术后每日累计仰卧时间。结果:两组患者术后舒适度评定结果显示试验组优于对照组,差异有统计学意义(P<0.05)。两组患者术前每日累计仰卧时间差异无统计学意义(P>0.05)。试验组术后当日及第1、2、3天每日累计仰卧时间均长于对照组,差异有统计学意义(P<0.05)。结论:使用头部固定枕可提高患者舒适度,延长每日累计仰卧时间,具有很好的临床实用价值。
        Obective: To design a economical and practical head fixed pillow to help patients keep supine position after Endothelial keratoplasty. Methods: Thirty patients(30 eyes) underwent endothelial keratoplasty from June 2014 to February 2017 in Renmin Hospital of Wuhan University were selected and divided by admission time into experimental group(with 15 patients, using new designed head fixed pillow) and control group(with 15 patients, using conventional method without any auxiliary tools after the operation). The comfortable degree, time of supine position and dislocation of transplant in patients were investigated by us in the two groups. Results: Compared with the control group, the experimental group had a higher degree of comfort(P<0.05). There were no statistical significance in the cumulative supine time in the two groups before surgery(P>0.05). However, the cumulative supine time in the experimental group was longer than that in the control group on the day of surgery and the next three days, the differences were statistically significant(P<0.05).Conclusion: This new designed head fixed pillowcan improves the degree of comfort and extends the cumulative supine time. The effect of the head fixed pillow is significant, with good clinical value.
