椎间孔镜辅助行髓核摘除术治疗腰椎间盘突出症疗效分析

摘要
目的探讨椎间孔镜辅助腰椎间盘突出症患者行髓核摘除术的临床效果。方法随机抽取本院2014年3月至2016年3月收治的腰椎间盘突出症患者78例,所有患者均采用髓核摘除术治疗,并按照手术方法的不同,将其分为研究组(椎间孔镜辅助下髓核摘除术37例)与对照组(小开窗髓核摘除术41例)。术后随访3个月,对比两组患者的治疗效果。结果所有患者均顺利完成手术,研究组出血量少于对照组,术后卧床时间短于对照组,住院时间短于对照组,差异有统计学意义(P<0.05);术后1周、术后1个月,研究组疼痛评分均低于对照组,Oswestry功能障碍指数均优于对照组,差异有统计学意义(P<0.05)。结论在腰椎间盘突出症患者治疗中,采用椎间孔镜辅助下髓核摘除术治疗的效果理想,具有创伤小、恢复快等特点,值得推广应用。
Objective To investigate the clinical effect of nucleus pulposusectomy in patients with lumbar intervertebral disc herniation. Methods A total of 78 patients with lumbar disc herniation treated at our hospital from March 2014 to March 2016 were randomly selected. All patients were treated with nucleus pulposus and were divided into study groups according to their surgical methods. Axillary microsurgery assisted nucleus pulpomy,n =37) and control group( small fenestration,n = 41). After 3 months of follow-up,the treatment effects of the two groups of patients were compared. Results All the patients completed the operation successfully. The study group had less bleeding than the control group. The bed rest time was shorter than that of the control group. The length of hospital stay was shorter than that of the control group. The difference was statistically significant( P < 0. 05). At 1 month after surgery,the pain score of the study group was lower than that of the control group,and the Oswestry dysfunction index was better than that of the control group. The difference was statistically significant( P < 0. 05).Conclusion In the treatment of patients with lumbar disc herniation,the use of transforaminal lens assisted nucleus pulposus removal is ideal for treatment. It has the characteristics of less trauma,faster recovery,and is worthy of popularization and application.
