2型糖尿病合并痛风患者常规生化指标应用研究

摘要
探讨常规生化指标检测在2型糖尿病合并痛风辅助诊断的价值分析。方法随机选择2015年3月至2017年4月,简阳市中医医院确诊为2型糖尿病合并痛风患者90例为观察组,选择同期2型糖尿病无痛风患者90例为对照A组,选择同期痛风无糖尿病患者90例作为对照B组,综合测定分析患者血清血糖(GLU)、血清尿酸(UA)、胆固醇(CHOL)、甘油三酯(TG)、高敏C反应蛋白(hs-CRP)、脂蛋白-α[Lp(α)]、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)。结果与对照A组比较,观察组患者的血清UA、CHOL、TG、hs-CRP、Lp(α)、LDL-C浓度水平均升高,差异有统计学意义(P<0.05),血清HDL-C浓度水平降低,差异有统计学意义(P<0.05)。与对照B组比较,观察组患者的血清GLU、UA、CHOL、TG、hs-CRP、Lp(α)、LDL-C浓度水平均升高,差异有统计学意义(P<0.05),血清HDL-C浓度水平降低,差异有统计学意义(P<0.05)。观察组患者的血清UA指标与GLU、CHOL、TG、hs-CRP、Lp(α)、LDL-C指标呈正相关(r=0.308、0.045、0.401、0.447、0.289、0.448,P<0.05),与HDL-C呈负相关(r=-0.503,P<0.05)。结论常规生化指标中血清UA、TG、hs-CRP、Lp(α)、HDL-C、LDL-C指标的检测有利于2型糖尿病合并痛风疾病的辅助诊断和病情监测及预后效果判断。
Objective To explore the clinical significance of serum biochemical markers in the diagnosis of type II diabetes along with gout disease. Methods 90 cases with confirmed type II diabetes and gout disease in our hospital from March 2015 to April 2017 were enrolled into the study.Meanwhile, 90 patients with type II diabetes alone and 90 patients with gout disease alone were also enrolled as control A and control B.Blood samples were collected and serum levels of GLU, UA, CHOL, TG, hs-CRP, Lp(a), HDL-C and LDL-C were determined and compared. Results The serum levels of UA, CHOL, TG, hs-CRP, Lp(a), LDL-C were significantly higher in patients with type II diabetes and gout disease compared to those of control A and control B(P<0.05), while the level of HDL-C was significantly lower(P<0.05).The serum level of GLU in patients with both diseases was similar to that of control A(P>0.05), but was higher compared to that of control B(P<0.05).The correlation analysis showed that, the serum UA in patients with both diseases was positively correlated with GLU, TG, hs-CRP, Lp(a), and LDL-C(r=0.308, 0.401, 0.447, 0.289, 0.448, P<0.05), while was negatively correlated with HDL-C(r=-0.503, P<0.05), there was no significant correlation with CHOL(P>0.05). Conclusions The detection of serum biochemical markers as UA, TG, hs-CRP, Lp(a), HDL-C and LDL-C can be helpful in the diagnosis of type II diabetes with gout disease as well as in the disease monitoring.
