双源CT双能量成像对痛风石诊断的临床研究

摘要
目的:探明双源CT双能量成像技术在诊断痛风患者四肢关节中尿酸盐沉积情况的诊断效果。方法:选取笔者所在医院2017年3月-2018年3月收治的45例疑似痛风患者作为观察组,15例具有下肢疼痛、类风湿关节炎等非痛风患者作为对照组。在分组完成后,将其进行混编并采用双源CT双能量成像处理软件进行系统扫描与处理后,交由三名专业医师进行独立评价与判断,对比观察组与对照组在不同部位的尿酸盐沉积结果并评估其临床评估中存在的具体差异。结果:通过对比后发现,观察组患者中分别有25、14、39例患者被检查出手腕、膝部及踝关节具有尿酸盐沉积情况,其中踝关节处沉积较为严重。对照组患者没有出现尿酸盐的沉积情况。根据双能量成像技术的显示结果,45例患者的痛风石针对中发现沉积390处,是临床病变估计率的4倍以上。结论:通过对比观察组与对照组最终痛风石的临床诊断结果,双源CT双能量成像在痛风石诊断临床应用中具有良好的效果,可以方便快捷地完成痛风患者尿酸盐的沉积检测工作,对于痛风石诊断与判别具有一定的应用价值,值得在临床中推广并借鉴。
Objective:To investigate the diagnostic effect of dual-source CT dual-energy imaging in the diagnosis of uric acid deposition in the limbs and joints of gout patients.Method:From March 2017 to March 2018,45 patients with suspected gout were selected as the observation group,and 15 patients with lower limb pain,rheumatoid arthritis and other non-gout patients as the control group.After the grouping was completed,the patients were mixed and scanned by dual-source CT dual-energy imaging processing software.Three specialists were assigned to make independent evaluation and judgment.The results of uric acid deposition in different parts of the observation group and the control group were compared and the differences in clinical evaluation were evaluated.Result:By comparison,the urate deposition in wrist,knee and ankle joint was found in 25 cases,14 cases and 39 cases respectively in the observation group,especially in ankle joint.There was no deposition of uric acid in the control group.According to the results of dual-energy imaging,390 deposits of gout stones were found in 45 patients,more than four times the estimated rate of clinical lesions.Conclusion:By comparing the clinical diagnostic results between the observation group and the control group,dual-source CT dual-energy imaging has a good effect in the clinical application of gout stone diagnosis.It is convenient and quick to complete the detection of uric acid deposition in gout patients.It has a certain application value in the diagnosis and discrimination of gout stone.It is popularized and used for reference in clinic.
