摘要
目的探究MRI诊断在子宫肌瘤和子宫腺肌瘤临床鉴别诊断中应用。方法方便选择该院2014年6月—2016年7月收治的58例高度疑似子宫肌瘤和子宫腺肌瘤患者,回顾性分析58例患者MRI影像学表现特征,分析MRI诊断子宫肌瘤和子宫腺肌瘤的准确性、敏感性和特异性。结果 58例患者中,32例为子宫肌瘤,26例为子宫腺肌瘤患者,MRI诊断子宫肌瘤和子宫腺肌瘤的准确性为91.37%、敏感性为75.00%和特异性为96.55%;子宫肌瘤MRI图像表现为T1WI呈稍低或低信号,T2WI呈高信号,子宫轮廓不光整,体积明显增大,边界有一层假包膜;子宫腺肌瘤子宫肌瘤MRI图像表现为T1WI和T2WI均呈杂斑点状高信号,T1WI和T2WI信号较为复杂,子宫轮廓光整,体积明显增大,病灶与子宫无明显边界,或边界不规则、模糊不清。结论 MRI检查可直接作为子宫肌瘤和子宫腺肌瘤的直接诊断依据,MRI图像表现特征特异性明显,在诊断中具有较高的准确性和敏感性,操作简单,具有较高的临床参考价值,值得在临床上推广应用。
        Objective To study the application of MRI in the differential diagnosis of fibroid and solenoma. Methods 58 cases of patients highly suspected with fibroid and solenoma admitted and treated in our hospital from June 2014 to July2016 were convenient selected, and the MRI imaging performance characteristics of them were retrospectively analyzed, and the accuracy, sensitivity and specificity of MRI in the diagnosis of fibroid and solenoma were analyzed. Results In the 58 cases, fibroid was in 32 cases and solenoma was in 26 cases, and the accuracy, sensitivity and specificity of MRI in the diagnosis of fibroid and solenoma were respectively 91.37%, 75.00% and 96.55%, and the MRI of fibroid was shown low or slightly low intensity in T1 WI and high intensity in T2 WI, the contour of uterus was unclear and the volume enlarged obviously with pseudo-capsule in the boundary, and the MRI of solenoma was shown the mottled and complex high intensity in T1 WI and T2 WI, the contour of uterus was smooth, and the volume enlarged obviously and there was no obvious boundary between lesions and uterus, or the boundary was irregular and unclear. Conclusion MRI examination can be used as the direct basis of fibroid and solenoma, and the specificity of MRI imaging performance characteristics is obvious with high accuracy and sensitivity in diagnosis, and the operation is easy with higher clinical reference value, which is worth promotion and application in clinic.
