摘要
目的探讨二维超声评分法联合超声造影在甲状腺结节良恶性鉴别诊断中的效果。方法选取2016年9月~2018年12月我院收治的390例疑似甲状腺结节患者,均采用二维超声评分法、超声造影进行诊断,将术后病理结果作为金标准,分析两种检查方法的临床价值。结果 390例患者中存在甲状腺结节数446个,其中148个恶性结节,298个良性结节。二维超声评分法的特异性为76.5%(228/298),灵敏度为90.5%(134/148),准确率为81.2%(362/446)。超声造影诊断的特异性为80.5%(240/298)、灵敏度为91.9%(136/148)、准确率为84.3%(376/446)。两种方法联合诊断的特异性为93.3%(278/298),灵敏度为91.9%(136/148),准确率为92.8%(414/446)。结论二维超声评分法联合超声造影可有效提升甲状腺结节患者的检出率,值得推广使用。
        Objective To investigate the effect of two-dimensional ultrasound scoring combined with contrast-enhanced ultrasonography in the differential diagnosis of benign and malignant thyroid nodules. Methods A total of 390 patients with suspected thyroid nodules treated in our hospital from September 2016 to December 2018 were selected, and diagnosed by two-dimensional ultrasound and contrast-enhanced ultrasound. The gold standard was postoperative pathology. The clinical value of the two examination methods was analyzed. Results There were 446 thyroid nodules in 390 patients, including 148 malignant nodules and 298 benign nodules. The two-dimensional ultrasound scoring method had a specificity of 76.5%(228/298), a sensitivity of 90.5%(134/148), and an accuracy of 81.2%(362/446). The contrast-enhanced ultrasound was 80.5%(240/298), the sensitivity was 91.9%(136/148), and the accuracy was 84.3%(376/446). The combined specificity of the two diagnostic methods was 93.3%(278/298), the sensitivity was 91.9%(136/148), and the accuracy was 92.8%(414/446). Conclusion Two-dimensional ultrasound scoring combined with contrastenhanced ultrasound can effectively improve the detection rate of patients with thyroid nodules, which is worthy of popularization.
