摘要
目的总结体外膜肺氧合(ECMO)联合持续肾替代疗法(CRRT)在爆发性心肌炎中的应用。方法中山市人民医院重症治疗科2010年1月至2015年12月收治16例使用ECMO治疗的暴发性心肌炎患者,其中12例并发急性肾衰竭,需联合CRRT治疗。回顾分析这12例患者的临床资料,包括患者基本情况、ECMO联合CRRT治疗前及治疗后72小时的生命体征及相关理化指标。结果 12例ECMO联合CRRT救治的暴发性心肌炎患者均存活出院,ECMO联合CRRT治疗72小时后,患者平均动脉压、CVP、CK、CKMB、Lac、NTpro BN、Creat、BUN、ALT、AST、TBill均较治疗前有显著改善(P<0.05)。结论 ECMO联合CRRT可为重症暴发性心肌炎患者提供有效支持治疗,提高存活率,改善预后。
Objective To investigate the experience of combine the extracorporeal membrane ECMO with CRRT for the treatment of fulminant myocarditis. Methods The clinical data of 12 patients who received ECMO combined with CRRT due to fulminant myocarditis between January 2010 and December 2015 were retrospectively analysed. Results All the 12 patients survived anddischarged.72 hours after the treatment of ECMO combined with CRRT, all indexes of patients were significantly improved than before treatment. Conclusion ECMO combined with CRRT can provideeffective support for the treatment of patients with fulminant myocarditis,improve survival rate,and mprove prognosis.
