漳州市妇女宫颈标本中人乳头瘤病毒的分子流行病学研究

摘要
目的:研究漳州市妇女宫颈标本中人乳头瘤病毒(HPV)的分子流行病学特征。方法:选取漳州市2015年1月-12月参加子宫颈癌筛查HPV感染者,对其采用反向寡核苷酸探针斑点杂交联合聚合酶链反应(PCR)技术,分析其基因亚型。结果:本次参加宫颈癌筛查为9677人,其中HPV为阳性1449例,总感染率为14.97%,患者平均年龄(39.8±8.7)岁。年龄小于25岁组感染率相比于其他年龄组,则高于后者且差异显著(P<0.05)。高危基因亚型前3位分别为HPV16、52和58,所占比例分别为22.43%、12.08%和11.25%;176例为2联及以上混合感染,所占比例为12.15%;混合感染前10位相应HPV基因亚型构成当中,主要为高危基因亚型;HPV多重基因亚型混合感染,自低年龄组至高年龄组,其发生率则表现出显著下降状况(P<0.05)。结论:漳州市女性人群具有较高的HPV阳性率,且于时期不同,差异表现不同;针对较为常见的高危亚型当中,占据主导地位的并非HPV18;低年龄组具有较高的多重基因亚型混合感染率及HPV感染率;在混合感染当中,主要为高危亚型联合其他基因型感染。
Objective:To study the molecular epidemiological characteristics of human papillomavirus(HPV) in cervical specimens of Zhangzhou city.Methods:The person who participated in in cervical cancer screening HPV infected were chose from Jan 2015 to Dec 2015,in the use of reverse oligonucleotide probe dot blot hybridization combined with polymerase chain reaction(PCR) technology,to analysis gene subtype Results:Totla cancer screening numbe was9677,of which HPV was positive in 1449 cases,the total infection rate was 14.97%,the average age of the patients(39.8 + 8.7) years old.Compared to other age groups,the age of 25 years old was higher than that of the latter and the difference was significant(P < 0.05).High- risk gene subtype of three were HPV16,52,58,the proportion of respectively 22.43%,12.08%and 11.25%:176 cases of 2 and above mixed infection,the proportion was 12.15%:mixed infection of the top 10 corresponding HPV subtypes among the composition,mainly for high- risk gene subtype:HPV gene subtype of mixed infection,since the low age group the highest age group.The rate showed a significantly decreased(P < 0.05).Conclusion:Zhangzhou city female population has higher positive rate of HPV,and in different periods,different performance:for more common high- risk subtypes and occupy the dominant position was not HPV18;low age group has a higher multiple genetic subtypes mixed infection and HPV infection rate:in mixed infection,mainly for high- risk subtypes in combination with other genotype infection.
