回肠袢式造口术和结肠造口术在老年梗阻性乙状结肠癌和高位直肠癌患者治疗中的安全性和有效性

摘要
目的探讨回肠袢式造口术和结肠造口术在老年梗阻性乙状结肠癌和高位直肠癌患者中治疗效果及安全性。方法选取2013年3月~2015年12月我院收治乙状结肠癌和高位直肠癌患者69例为研究对象,根据患者手术方式不同分为A组(回肠袢式造口术)和B组(结肠造口术),两组患者均行Ⅰ、Ⅱ期手术治疗,比较两组患者手术时间、术中出血量、胃肠功能恢复时间、进食时间、住院时间及不良反应发生率。结果 A组患者手术时间、胃肠功能恢复时间、进食时间及住院时间均短于B组患者,比较有统计学意义(P<0.05)。A组患者肠造口异常并发症发生率低于B组,比较有统计学意义(P<0.05)。结论回肠袢式造口术在老年梗阻性乙状结肠癌和高位结肠癌患者中应用可缩短患者治疗时间和康复时间,相对结肠造口具有更高疗效和治疗安全性。
Objective Discussion ileum colostomy and loop colostomy therapeutic efficacy and safety in elderly obstructive sigmoid colon cancer and high colorectal cancer patients. MethodSelect March 2013 ~ December 2015 in our hospital sigmoid colon cancer and colorectal cancer high 69 patients for the study, according to surgical patients were divided into group A(ileum loopcolostomy) and group B(colostomy surgery), two groups of patients were stage ⅰ, ⅱ surgery were compared operative time, blood loss, gastrointestinal function recovery time, eating time, hospitalizationand incidence of adverse reactions. Result A group of patients surgery time, recovery time of gastrointestinal function, eating time and hospital stay were shorter in group B patients was statisticallysignificant(P <0.05). A group of patients with abnormal intestinal stoma complication rate is lower than group B, it was statistically significant(P <0.05). Conclusion Ileal loop colostomy applicationcan shorten the treatment time and patient recovery time in elderly obstructive sigmoid colon cancer and colon cancer patients in high relative colostomy with higher efficacy and safety of treatment.
