宫环出血病证动物模型的建立及其发病机理的实验研究

摘要
目的:探讨宫环出血病证动模型的建立方法,并在此基础上进行其发病机理的研究。
     方法:根据中医理论建立了置Cu-IUD家兔气虚血瘀、肝郁血瘀、阴虚血瘀三个证型的动物模型,并分别观察了血液流皮学的变化、动物一般活动的改变和体重变化、主要脏器的平均重量、肝细胞超微结构、血浆环核苷酸(cAMP、cGMP)水平及其比值等多个方面。
     结果:①宫内置Cu-IUD能升高全血粘度、血浆粘度、红细胞压积水平,加快血沉速度,使血液呈高凝状态。
     ②各组动物的一般活动均有改变,体重均不同程度的减轻。
     ③气虑血瘀型家兔的主要脏器均有不同程度的萎缩;而肝郁血瘀型、阴虚血瘀型家兔的脏器无明显改变。
     ④肝郁血瘀型家兔的肝细胞超微结构受损;而气虚血瘀型、阴虚血瘀型家兔的肝细胞结构接近正常。
     ⑤阴虚血瘀型家兔的血浆环磷酸腺苷(cAMP)水平升高,cGMP无明显变化,cAMP/cGMP值增加;气虚血瘀型、肝郁血瘀型家兔的血浆cAMP降低,cGMP升高,cAMP/cGMP值降低。
     结论:上述实验结果表明,由于个体的差异,各证型之间存在不同的表现,但金韧异物直接损伤胞宫是宫环出血发病机理的本质,瘀血阻络为主要发病机理,环致血瘀是其根本病因。
Objective: To search for a method of building up the animal model with GongHuanChuXue(GHCX) diseases and TCM syndromes. Moreover, to study the pathogenesis of GHCX on the model.
    Methods: Based on the TCM theory, three Cu-IUD-wearing rabbit models of QiXuXueYu syndromes(deficiency of vital energy and stagnation of blood), GanYuXueYu syndromes(depression of the liver and stagnation of blood) and YinXuXueYu syndromes(deficiency of Yin and stagnation of blood) were builded up. Several items such as the change of hemorrheologic, the change of general activity and body weight , the mean weight of main organs ,the ultrastructures of hepatocytes, the cyclic nucleotide acid in plasma and their ratio were observed.
    Results: (1)Wearing Cu-IUD could bring blood into the hypercoagulable state for it could increase the level of total blood viscosity , plasma viscosity, red cell volume, and accelerate the speed of erythrocyte sedimentation. (2)All animals had the alteration of general activity and body weight lossing in variant degrees.
    
    
    (3)The main organs in QiXuXueYu group all had atrophied in variant degrees, while the GanYuXueYu group and YinXuXueYu group had no obvious change. (7)The hepatocyte ultrastructures in the GanYuXueYu group were impaired, while it hadn't visible change in the QiXuXueYu group and YinXuXueYu group. (5) The level of plasma cyclic adenosine monophosphate (cAMP) in the YinXuXueYu group increased, while cyclic guanosine monophosphate(cGMP) had no obvious change, so the cAMP/cGMP ratio raised. Contrarily, the level of cAMP in the QiXuXueYu group and GanYuXueYu group decreased, while cGMP increased, so the ratio of cAMP/cGMP declined.
    Conclusion: All above indicated that different model had various phenotype because of the individual difference. The nature of GHCX's pathogenesis was the impairment by the metal foreign material directly and the main pathogenesis of GHCX was the stagnation of blood in vessel. Blood stagnation induced by the IUD was the fundamental etiology of GHCX.
