汶川地震救援消防官兵和医护人员创伤后应激障碍远期患病的中间变量分析

摘要
目的了解某部救援消防官兵和医护人员创伤后应激障碍(PTSD)远期患病与应激中间变量相关性的差异。方法采用问卷调查及多元线性回归方法对完成救援1年后的消防官兵32例、医护人员38例进行PTSD患病与应激中间变量的相关性分析。结果两组PCL-C各项因子均与精神质和认知评价相关。消防官兵组PCL总分及警觉性增高与理想化正相关,创伤再体验与投射正相关。医护人员组PCL总分与掩饰度负相关,创伤再体验与负性认知、否认、假性利他正相关,与回避负相关,警觉性增高与伴无能之全能负相关。结论 消防官兵和医护人员PTSD远期患病与中间变量的相关性存在明显差异。
Objective To determine the difference in the correlation of long-term prevalence of post traumatic stress disorder(PTSD) and intermediate variables between the firemen and medics after Wenchuan earthquake rescuing.Methods The 32 firemen and 38 medics were interviewed using questionnaires one year after earthquake rescuing.A correlation analysis was made between PTSD prevalence and intermediate variables by multiple linear regression.Results The PCL score correlated with P(psychoticism) and cognitive evaluation both in firemen and medics.In the firemen group,the PCL score and hyperarousal correlated positively with idealization;re-experiencing correlated positively with projection.But in the medics group,the PCL score correlated negatively with L(lie);re-experiencing correlated positively with negative cognition,denial,pseudo-altruism,and negatively with avoidance;hyperarousal correlated negatively with omnipotent feeling accompanied by impotence.Conclusion There is obviously a difference in the correlation of long-term PTSD prevalence and intermediate variables between the firemen and medics one year after earthquake rescuing.
