咽食管Killian-Jamieson憩室的超声诊断分析

摘要
目的探讨超声诊断咽食管Killian-Jamieson憩室方法和流程。方法收集Killian-Jamieson憩室14例,回顾性分析超声声像图特征,对误诊原因和诊断流程综合分析。结果憩室位于甲状腺左叶背侧10例,右叶背侧4例,当地医院均超声误诊为甲状腺结节,拟行穿刺检查。患者患侧卧位饮水时,超声显示体积增大4例。食管X线钡餐检查证实为食管憩室。结论超声检查中嘱患者患侧卧位饮水,观察病灶内部回声变化是提高Killian-Jamieson憩室诊断的有效方法,超声检查结合食管X线钡餐造影可避免食管憩室误诊和对患者不必要的穿刺及无计划的外科手术。
Objective To explore the method and process of ultrasonography diagnosis of pharyngeal esophageal killian-jamieson diverticulum.Methods A total of 14 cases of killian-jamieson diverticulum were collected,and ultrasonographic features were analyzed retrospectively.Results This group of diverticulum was located in the dorsal side of the left lobe of the thyroid gland in 10 cases and the dorsal side of the right lobe in 4 cases.When the patients were drinking water in the lateral decubitus position,the ultrasound showed an increased of volume in 4 cases.These were confirmed as esophageal diverticulum by Esophageal X-ray barium meal examination.Conclusions It is an effective method to improve the diagnosis of killian-jamieson diverticulum by asking patients to drink water in the lateral decubitus position during ultrasound examination,and to observe the changes of echo in the lesions.Ultrasound examination combined with esophageal X-ray barium meal imaging enable avoid misdiagnosis of esophageal diverticulum and unnecessary FNA and unplanned surgical treatments for patients.
