弥散张量成像对遗忘型轻度认知障碍的诊断及预后评估价值

摘要
目的:探讨遗忘型轻度认知障碍的DTI图像表现,并判断其预后。资料与方法:选取2012年至2015年来我院就诊的轻度认知障碍的患者61例,其中男32例,女29例。在社区招募健康受试者60例,男34例,女26例。对两组病人做神经心理测试,MRI一般序列扫描及DTI,测定期FA值,并随访一年,观察aMCI患者的转归情况。运用SPSS软件对FA值进行X2检验,观察其有无统计学差异。结果:61例aMCI患者中有15例12个月后转化为AD,正常组与aMCI组的额叶、颞叶、顶叶、枕叶、海马、下额枕部纤维束、胼胝体膝部、胼胝体压部、上纵束的FA值,没有显著地统计学差异;在扣带束的FA值的差异具有统计学意义。发展为AD的aMCI组与未发展为AD的aMCI组,在额叶、颞叶、海马、下额枕部纤维束、胼胝体膝部、扣带束的FA值的差异具有统计学意义;在顶叶、枕叶、胼胝体压部、上纵束的FA值无显著统计学意义。结论:DTI对区分aMCI类型及判断其预后具有重要价值。
