摘要
目的探讨小红细胞和RDW对Sysmex XE-5000血细胞分析仪血小板计数的影响。方法选取中山大学孙逸仙纪念医院2018年3月至12月期间,住院患者标本240例,测定红细胞平均体积(MCV)和红细胞分布宽度(RDW),根据两者不同范围进行分组,记录PLT-I、PLT-O、RDW-CV三项指标的结果,并对所测标本用显微镜手工法进行血小板计数,最后通过SPSS19.0统计软件行配对t检验统计分析。结果 (1)当MCV为70~80fL、RDW-CV<15%时电阻抗法(PLT-I)与手工法计数血小板结果比较差异无统计学意义;MCV<60fL、MCV为60~69fL时电阻抗法(PLT-I)检测结果均高于手工法血小板计数,差异有统计学意义;MCV为70~80fL且RDW-CV≥15%时,电阻抗法(PLT-I)与手工法计数血小板的差异有统计学意义(P<0.01)。(2)当MCV<60fL、MCV为60fL~69fL、MCV为70fL~80fL时,血小板检测结果荧光法(PLT-O)均与手工法相近,差异无统计学意义(P>0.05)。结论小红细胞和RDW-CV大小对电阻抗法(PLT-I)影响较大,而荧光法(PLT-O)受二者影响较小,可以更准确反映血小板真实水平。
        Objective To investigate the effect of small red blood cells and RDW on platelet count of Sysmex XE-5000 hematology analyzer. Methods A total of 240 inpatient specimens were selected from Sun Yat-sen Memorial Hospital of Sun Yat-sen University from March to December 2018. The mean red blood cell volume(MCV) and red blood cell distribution width(RDW) were determined and grouped according to the different ranges. PLT-I was recorded. The results of the three indicators, PLT-O, RDW-CV, and platelet counts were performed by microscope manual method. Finally, the paired t-test statistical analysis was performed by SPSS19.0 statistical software. Results(1) When the MCV was 70-80 fL and RDW-CV<15%, there was no significant difference between the electrical impedance method(PLT-I) and the manual counting platelet count. When MCV<60 fL and MCV were 60~69 fL The results of electrical impedance spectroscopy(PLT-I) were higher than those of manual platelet counts, and the difference was statistically significant. When MCV was 70-80 fL and RDW-CV ≥15%, electrical impedance method(PLT-I) and manual counting were used. Platelet differences were also statistically significant(P<0.01).(2) When MCV<60 fL, MCV was 60 fL~69 fL, MCV was 70 fL~80 fL, the platelet detection result fluorescence method(PLT-O) was similar to the manual method, and the difference was not statistically significant(P>0.05).Conclusion The size of small red blood cells and RDW-CV has a great influence on the electrical impedance method(PLT-I), while the fluorescence method(PLT-O) is less affected by both, which can more accurately reflect the true level of platelets.
