128例中重度慢性牙周炎患者下颌磨牙牙半切除术后固定修复治疗效果的观察

摘要
目的观察128例中重度慢性牙周炎患者的下颌磨牙行牙半切除术后固定修复治疗的效果。方法选取128例中重度牙周炎患者的下颌磨牙,完善根管治疗和牙周基础治疗后行牙半切除术,分成四组行固定桥,联冠和单冠修复,于术前,术后6个月,18个月,30个月观察余留牙的牙周探诊深度(Probing depth, PD)和修复成功率。结果牙半切术后各组牙周探诊深度均较术前有明显变化,差异有统计学意义(P<0.05);各组牙周探诊深度同半切术后6个月比较,差异有统计学意义(P<0.05);各组间18个月内修复体成功率差异无统计学意义,但30个月后各组修复体成功率差异有统计学意义(P<0.05)。结论中重度牙周炎患者下颌磨牙牙半切除术后优先选择双端固定桥和联冠修复。
Objective To observe the effect of different fixed restorations after hemisection of mandibular molar in 128 patients with severe and moderate chronic periodontitis. Methods After the perfect root canal therapy and periodontal initial therapy were completed,the mandibular molars of 128 patients with moderate and severe chronic periodontitis were treated by hemisection. Then divided into four groups using rigid fixed bridges,united crowns and single crowns,the probing depth and repair success rate of remaining teeth were observed before and after operation in 6 months, 18 months and 30 months. Results The probing depth of each group after hemisection was significantly changed compared with preoperative, and the difference was statistically significant(P<0.05).The index of probing depth in each group was significantly different from that in 6 months after operation(P<0.05). There was no statistically significant difference in the success rate of prosthesis within 18 months between groups, but the success rate of prosthesis in each group was statistically significant after 30 months. Conclusion For the patients with moderate and severe periodontitis, the choice of rigid fixed bridge is the best choice for mandibular molars treated by tooth hemisection.
