抗环瓜氨酸肽抗体、类风湿性因子联合检测在类风湿性关节炎诊断中的应用价值

摘要
目的探讨抗环瓜氨酸肽(抗-CCP)抗体、类风湿因子(RF)联合检测对类风湿关节炎(RA)的临床应用价值。方法选取类风湿性关节炎患者117例为类风湿关节炎组,另选取其他自身免疫病患者86例和正常健康体检者120例为对照组。检测抗-CCP、RF均采用免疫比浊法。结果 RA组的抗-CCP和RF的阳性率均高于非RA组和对照组,差异有统计学意义(P<0.05)。RA组抗-CCP和RF二者联合检测的阳性率明显高于其余2组,差异有统计学意义(P<0.05),抗-CCP灵敏度为82.4%、特异度为93.5%,RF灵敏度为84.8%、特异度为76.4%;二者联合检测,灵敏度为82.4%、特异度为94.5%。结论 RF与抗-CCP抗体联合检测有利于提高RA检测的阳性率,对RA诊断及早期诊断具有重要的临床意义。
        Objective To investigate the combined detection value of anti cycliccitrullinated peptide(CCP) antibody and rheumatoid factor(RF) in the diagnosis of rheumatoid arthritis(RA). Methods A total of 117 patients with rheumatoid arthritis(RA) were selected as the control group, and patients with other autoimmune diseases(n=86) and normal healthy subjects(n=120) were selected as control group. The anti-CCP and rheumatoid factors were detected by immune turbidimetry. Results The positive rates of anti CCP and RF in RA group were higher than those of autoimmune group and control group(P<0.05). The positive rate of combined detection in the RA group was significantly higher than the other two groups, with the differences statistically significant(P<0.05); The sensitivity of anti CCP antibody was 82.4%, specificity was 93.5%; The sensitivity of RA was 84.8%, the specificity was 76.4%; The sensitivity of combined detection was 82.4%, specificity was 94.5%. Conclusion RF combined with anti-CCP antibody is helpful to improve the positive rate of RA detection, and has important clinical significance for the early diagnosis of RA.
