早孕期超声规范化结构筛查在胎儿四肢畸形诊断中的应用分析

摘要
目的:探究早孕期超声规范化结构筛查在胎儿四肢畸形诊断中的应用价值。方法:研究时间:2017年8月—2018年1月,选取笔者所在医院进行产前超声规范化结构检查的早孕期孕妇782例(11~13~(+6)孕周),同时进行颈部透明带(NT)进行筛查,所有检查异常孕妇,随访观察至引产或分娩,分析其诊断结果。结果:经腹彩色多普勒超声进行检查后,图像先显示:单胎妊娠中出现3例肢体畸形、双胎妊娠出现1例肢体畸形,四肢畸形检出率为0.51%。图像特征:1例双下肢股骨缺陷;1例左侧下肢缺如,1例左侧股骨缺如伴脚趾异常,并伴有皮下水肿以及颈部淋巴水囊瘤;1例尺桡骨发育缺陷,且合并有脊柱裂以及脐带膨出等现象。结论:针对早孕期孕妇,采用超声规范化结构筛查的准确度较高,可在孕早期及早发现胎儿肢体畸形情况,对临床及早引产治疗具有一定的指导作用,尽量避免了妊娠孕周较大孕妇的引产率,对双胎以及胚胎移植胎儿的处理具有一定的临床应用价值,可在临床上推广应用。
        Objective To explore the value of ultrasonographic standardized structure screening in the diagnosis of fetal extremity malformation in early pregnancy. Methods Study time: From August 2017 to January 2018, we selected 782 cases(11 ~ 13~(+6) weeks of gestational age) of early pregnancy pregnant women who were in the author's hospital for prenatal ultrasound standardized structure examination, and also performed the cervical transparent zone(NT). Screening was performed. All pregnant women with abnormal findings were followed up until induction of labour or childbirth, and their diagnosis was analyzed. Results After examination by color Doppler ultrasound, the images showed first: 3 cases of limb deformity in a single fetal pregnancy, 1 case of twin pregnancy and 1 limb deformity, and the detection rate of limb deformity was 0.51%. Image features: 1 case of femoral defects in both lower extremities; 1 case of left lower extremity absence, 1 case of left femur absent with toe abnormalities, accompanied by subcutaneous edema, and lymphatic sac tumor in the neck; 1 case of ulnar-ulnar bone development defect, and Including spina bifida and umbilical cord bulging. Conclusion For pregnant women during the first trimester, the accuracy of screening by ultrasonic standardization structure is high, and it can be used to detect fetal malformation early in the first trimester of pregnancy. It has a certain guiding effect on early induction of labor, and avoids pregnant women with large gestational age during pregnancy. The induction rate has certain clinical application value for the treatment of twins and fetuses that have been transferred by embryos, and can be popularized and applied in clinical practice
