电针曲池、上巨虚等穴对UC大鼠治疗效应的比较探讨“合治内府”的涵义

摘要
目的:比较电针大肠下合穴足阳明经的上巨虚与其本经五输穴中的合穴曲池以及足阳明胃经下合穴足三里在治疗溃疡性结肠炎(Ulcerativecolitis,UC)模型大鼠中的效应,试图探讨“合治内府”中合穴的涵义,为阐明“合治内府”理论提供实验依据。
     方法:将50只雄性Wistar大鼠完全随机分为5组:正常组、模型组、上巨虚组、足三里组、曲池组,每组10只。三硝基苯磺酸(trinitrobenzenesulfonic acid,TNBS)法造模后,观察电针上巨虚、足三里、曲池后UC大鼠结肠黏膜损伤(炎症、溃疡)程度评分、髓过氧化物酶(myeloperoxidase,MPO)含量、白介素-1β(interleukin-1β,IL-1β)含量、白介素-4(interleukin-4,IL-4)含量的变化。
     结果:1.电针“上巨虚”、“足三里”穴使UC大鼠结肠黏膜损伤有不同程度的改善,与“模型”组、“曲池”组比较差异有显著性(P<0.05)。
     2.电针“上巨虚”、“足三里”、“曲池”穴能使UC大鼠结肠组织MPO活性不同程度的降低,其中“上巨虚”、“足三里”与“模型”组比较差异有显著性(P<0.05或P<0.01),“曲池”组虽有降低趋势,但无统计学意义(P>0.05)。
     3.电针“上巨虚”、“足三里”、“曲池”穴使UC大鼠结肠组织IL-1β含量有不同程度的降低,“上巨虚”、“足三里”与“模型”组比较差异有显著性(P<0.05),“曲池”组与“模型”组比较无统计学意义(P>0.05)。
     4.电针“上巨虚”、“足三里”、“曲池”穴使UC大鼠结肠组织IL-4含量有不同程度的升高;“上巨虚”、“足三里”与“模型”组、“曲池”组比较差异有显著性(P<0.05或P<0.01),“曲池”组与“模型”组比较比较差异无显著性(P>0.05)。
     5.综观上述各项指标,比较电针“上巨虚”与“足三里”,其差异无统计学意义(P>0.05)。
     结论:1.电针上巨虚、曲池对UC大鼠结肠黏膜组织形态学指标与MPO活性的影响,可以看出电针上巨虚对UC具有明显的效应,表明大肠下合穴上巨虚与大肠腑有着相对特异性的联系。
     2.电针上巨虚可使IL-1β的含量出现降低,而使IL-4的含量出现上升,提示调节促炎细胞因子IL-1β和抑炎细胞因子IL-4的含量可能是上巨虚实现对UC发挥治疗作用的重要环节之一。
     3.对比上巨虚与曲池对UC的治疗效应,大肠腑下合穴上巨虚与大肠经合穴曲池对MPO、IL—4的调节作用上均存在明显差异,提示“合治内府”理论的合穴应为下合穴,而不是上合穴或上、下合穴的合称。
     4.对比上巨虚、足三里对UC上述观察指标中的治疗效应,二者无明显差异,可能与二穴在大鼠下肢相距的部位或胃肠的生理病理机制相关,尚待进一步研究。
Objectives:The theory of "curing viscera diseases by He points" was studied by comparing the electrotherapy effect of Shangjuxu(lower He point of large intestine fu),Zusanli(lower He point of stomach),and Quchi(He point of large intestine meridian) on treating ulcerative colitis(UC) rats,trying to know the mean of He point in "curing viscera diseases by He points".It offered some experimental evidence to clarifying the theory of "curing viscera diseases by He points".
     Methods:Fifty male Wistar rats were randomly assigned into five groups: Normal group(A)、Model group(B)、Shangjuxu point group(C)、Zusanli point group(D)、Quchi point group(E),each group had ten rats.Made UC model rats with trinitrobenzene sulfonic acid,observing the change of the colon tectology、active value of myeloperoxidase(MPO)、the content of interleukin-1beta(IL-1β) and interleukin-4(IL-4) of the UC rats colon mucosa after being electrotherapy on Shangjuxu、Zusanli and Quchi points.
     Results:1.Electrotherapy Shangjuxu、Zusanli can improve UC rats' colon histomorphology index in different levels.Comparing with B、E,there was significantly different(P<0.05) 2.Electrotherapy Shangjuxu、Zusanli、Quchi could decrease MPO activity in tissue of colon mucosa of UC rats in different levels.Comparing with B,there was obviously different in C and D(P<0.05 or P<0.01);in E even it had decreased,but there was no statistics significance(P>0.05).3.Electrotherapy Shangjuxu、Zusanli、Quchi can decrease content of IL-1βin tissue of colon mucosa of UC rats in different levels.Compared with model group,there was obviously different in C and D (P<0.05);but there was no statistics significance in E(P>0.05).4. Electrotherapy Shangjuxu、Zusanli、Quchi can increase content of IL-4 in tissue of colon mucosa of UC rats in different levels.Comparing with B,there is obviously different(P<0.05 or P<0.01);in E even it had decreased,but there was no statistics significance(P>0.05).5.Making a comprehensive view of all indexes above,there is no statistics significance between C and D.
     Conclusions:1.ComParing with the morphological change and the value of MPO of UC rats in Shangjuxu and Quchi,a significant effect on UC could be seen in electrotherapy Shangjuxu,it indicated its relative correlation with large intestine fu.2.Electrotherapy Shangjuxu could decrease promotion of inflammation of IL-1β,while increase promotion anti- inflammation of IL-4. It Probably is one of the important mechanisms of electrotherapy on C to treat UC.3.Comparing the effect and the mechanism of electrotherapy Shangjuxu and Quchi to treat UC rats,there was significally difference on regulating MPO and IL-4 between Shangjuxu(lower He point of large intestine fu) and Quchi(He point of large intestine meridian),it showed that He point of the theory of "curing viscera diseases by He points" means lower He point,but not He point of meridian or floorboard of lower He point and He point of meridian. 4.Comparing the effect and the mechanism in all indexes of electrotherapy Shangjuxu and Zusanli to treat UC rats,there was not significance different.It maybe related with two points on the rats lower limbs' place or the relationship of physiological and pathological on stomach and intestines.It needs farther research.
