We recorded somatosensory evoked potentials to median nerve stimulation during isoflurane or sevoflurane induced EEG burst suppression and continuous suppression. The P14 is readily and reliably recorded with nasopharyngeal electrode even during continuous EEG suppression. Its latency increases and amplitude decreases but less than that of the N20 wave. The N20 is reduced to a monophasic, negative wave which can still be recorded during continuous EEG suppression. The precentral P22 generator is probably also active during EEG suppression as the precentral positive wave peaks later and has a much wider distribution than the postcentral N20 wave. During EEG suppression stimuli to median nerve are regularly followed by the N20/P20 (P22) wave which may exceed 10 μV in amplitude in derivations like P3-C3, and is readily seen in single sweeps because background EEG is suppressed. This provides another means in addition to stimulus-evoked bursts to clinical somatosensory single-sweep monitoring during EEG burst suppression. The monotonous increase in latency and decrease in amplitude make the P14 and N20 useful indicators of the depth of anaesthesia still when the EEG is continuously suppressed.
