摘要

目的:探讨不典型胎盘早剥的临床特点,提高对不典型胎盘早剥的认识及早期诊断率。方法:收治不典型胎盘早剥患者21例,回顾性分析其临床资料。结果:21例患者中,9例有阴道出血,17例出现腹痛或腹部不适症状,5例腹部触诊有固定压痛点,2例有胎心异常,2例出现子宫张力增高,宫缩频率明显增强。仅13例为临床确诊,另8例为产后胎盘检查后回顾性诊断,所有孕妇未出现不良妊娠结局。结论:不典型胎盘早剥临床表现隐匿,早期临床确诊困难,应提高认识,加强监测并反复检查,才能确保母婴安全。
        Objective:To investigate the clinical features of atypical placental early stripping and to improve the understanding and early diagnosis of atypical placental early stripping.Methods:21 patients with atypical placental early stripping were selected.The clinical data were retrospectively analyzed.Results:Among 21 cases,9 cases had vaginal bleeding;17 cases had abdominal pain or abdominal discomfort;5 cases had fixed tenderness point in abdominal palpation;there were 2 cases with fetal heart abnormality;2 cases had uterine hypertonia,and contraction frequency was significantly enhanced.Only 13 cases were diagnosed clinically,and the other 8 cases were retrospective diagnosis after postpartum placental examination.All pregnant women did not appear adverse pregnancy outcome.Conclusion:The clinical manifestations of atypical placental early stripping were occult,and early clinical diagnosis was difficult.Raising awareness,strengthening monitoring and repeated checks can ensure the safety of mother and infant.
