基于数据挖掘的刘启泉治疗消化性溃疡用药规律研究

摘要
目的分析刘启泉教授治疗消化性溃疡的用药特点及规律,为该病临床治疗提供参考。方法整理刘教授2016年10月-2017年10月于河北中医学院附属医院脾胃病科门诊治疗的消化性溃疡有效医案。将方药录入中医传承辅助平台(V2.5),对药物频次、性味、归经、关联规则及熵聚类的新处方进行分析。结果纳入处方158首,涉及药物214味,其中使用频次≥20次的药物33味,包括延胡索、白芍、地榆、三七粉、蒲公英等。处方药物以温、寒、平性及辛、甘味为主,主归脾、肝、胃经。挖掘得到高频药物组合69组、关联规则40条、核心药物组合16组、新处方8首。常用药物配伍有延胡索-白芍、地榆-三七粉、蒲公英-预知子。结论刘教授治疗消化性溃疡以疏肝理气、清热化瘀、和胃止痛为主要治法;"调气之通,理气行气,谨防伤阴""清热之通,慎用苦寒,巧施甘寒""祛瘀之通,疏气散瘀,气血并调"为其主要用药特点。
Objective To analyze medication characteristics and rules of Professor LIU Qiquan for treatment of peptic ulcer(PU); To provide references for clinical treatment of the disease. Methods Effective prescriptions for PU in Outpatient of Department of Spleen and Stomach Diseases, Affiliated Hospital of Hebei College of Traditional Chinese Medicine treated by Professor LIU from Oct. 2016 to Oct 2017 were collected. The prescriptions were introduced into TCM Inheritance Auxiliary Platform(V2.5). The medicine taste, meridians, use frequency, medicine association rules and entropy of new prescriptions were analyzed. Results Totally 158 prescriptions were included, involving 214 kinds of Chinese materia medica, among which the use frequency of 33 kinds of Chinese materia medica ≥20, including Corydalis Rhizoma, Paeonize Radix Alba, Sanguisorbae Radix, Notoginseng Radix et Rhizoma Powder, and Taraxaci Herba. The Chinese materia medica in prescriptions were mainly warm, cold, neutral, spicy and sweet flavors, mainly belonging to spleen, liver and stomach meridians. Data mining obtained 69 groups of high-frequency medicine combinations, 40 association rules, 16 core medicine combinations, and 8 new prescriptions. The common medicine combinations were Corydalis Rhizoma-Paeonize Radix Alba, Sanguisorbae Radix-Notoginseng Radix et Rhizoma Powder, and Taraxaci Herba-Akebiae Fructus. Conclusion The main treatment principles of Prof. LIU for PU are relieving qi stagnancy in liver, clearing heat and reducing stasis and relieving pains in stomach. The main medication characteristics are "regulating qi and promoting qi and not harming yin", "clearing heat, carefully using bitter and cold medicine, using sweet and cold medicine flexibly", and "relieving stasis, relieving qi stagnancy, balancing qi and blood".
