不同入路人工全髋关节置换术治疗高龄老年股骨颈骨折的短期疗效观察

摘要

目的探讨SuperPATH入路及后外侧入路人工全髋关节置换术治疗高龄老年股骨颈骨折的短期疗效。方法我院收治的92例高龄股骨颈骨折患者,按照手术入路方式的不同分为SuperPATH入路组及后外侧入路组,记录切口长度、术中出血量、手术时间、术后引流量、术后下地时间、住院时间及术后并发症发生情况;于术后2周及术后1、3个月进行髋关节评分。结果后外侧入路组手术时间短于SuperPATH入路组,术后下地时间及住院时间长于SuperPATH入路组,切口长度、术中出血量、术后引流量高于SuperPATH入路组,术后2周及1个月时髋关节功能评分低于SuperPATH入路组(P <0. 05);两组术后并发症发生情况比较差异无统计学意义(P> 0. 05)。结论经SuperPATH全髋关节置换术对高龄股骨颈骨折患者具有更好的短期疗效。
        Objective To investigate the short-term effect of total hip arthroplasty with Super PATH approach and posterolateral approach in the treatment of femoral neck fracture in elderly patients. Methods Ninety-two patients with femoral neck fractures in our hospital were selected in this study. The patients were divided into Super PATH approach group and posterolateral approach group according to the different surgical approaches.The length of the incision,intraoperative blood loss,operation time,postoperative drainage,postoperative ground time,length of hospital stay,and postoperative complications of the patients were recorded.The hip scores were performed at 2 weeks,1 month and 3 months after the surgery. Results The operation time was significantly shorter,the postoperative ground time and length of hospital stay were significantly longer,and the incision length,the amount of hemorrhage and postoperative drainage in the posterolateral approach group than those in the Super PATH approach group( all P < 0. 05).The hip function scores of the posterolateral approach group were significantly lower than those of the Super PATH approach group at 2 weeks and 1 month after surgery( P < 0. 05).There was no significant difference in postoperative complications between the two groups( P > 0. 05).Conclusion The total hip arthroplasty with Super PATH has a better short-term effect in the treatment of patients with femoral neck fractures.
