南宁市登革热大流行后人群中抗原抗体的变化

摘要
目的了解南宁市登革热大流行后人群中抗原抗体的变化情况。方法 2015—2018年在南宁中心血站随机抽取2 821名献血者的静脉血5~6 mL于EDTA抗凝管中,酶联免疫吸附试验(ELISA)检测DENV NS1抗原、IgM抗体和IgG抗体。分别用Logistic回归和秩和检验方法分析采样年份与抗原抗体定性检测结果和A_(450 nm)值之间的关联性。结果 2015—2018年各年均有DENV的新发感染者和既往感染者。定性检测中,DENV NS1抗原的检测结果与采样年份无关(P>0.05),而DENV IgM抗体(P <0.05)和IgG抗体(P <0.05)的检测结果与采样年份有关;2018年IgM阳性率显著高于2016和2017年;2018年IgG阳性率显著低于2015、2016和2017年。A_(450 nm)值检测中,DENV NS1抗原(P <0.0083)、IgM抗体(P <0.0083)、IgG抗体(P <0.0083)的检测结果均与采样年份有关;NS1抗原的A_(450 nm)值由高到低依次是2017、2018、2015和2016年;IgM抗体的A_(450 nm)值由高到低依次是2018、2016和2017、2015年;IgG抗体的A_(450 nm)值由高到低依次是2015、2016和2017、2018年。结论南宁市在登革热大流行过后一直存在疾病的流行,且随着时间的延长,因大流行所获得的被动保护作用逐渐下降,2018年起人群的易感性明显增加。
        Objective To understand the change situation ofantigens and antibodies of DENV after dengue epidemic in Nanning. Methods A total of 2 821 blood donors were random selected in Nanning blood center from 2015 to 2018 and their 5-6 mL blood samples were collected. DENV NS1 antigen, IgM and IgG antibodies were detected with enzyme linked immunosorbent assay(ELISA).The relevance between the year of sample collection and the detection results were analyzed with logistic regression models,while the relevance between the year of sample collection and the value of A_(450 nm) were analyzed with rank sum test. Results There were infected persons who been infected in past and in present by DENV every year from 2015 to 2018. In qualitative detection, there was irrelevance between the detection result of NS1 and the year of sample collection(P > 0.05), while there was relevance between the detection result of IgM(P < 0.05) or IgG(P < 0.05) and the year of sample collection. The prevalence of IgM antibody was significantly higher in 2018 than that in 2016 and 2017. The prevalence of IgG antibody was significantly lower in 2018 than that in 2015, 2016 and 2017. In the detection of A_(450 nm), there was relevance between the detection value of NS1(P < 0.008 3), IgM(P < 0.008 3) or IgG(P < 0.008 3) and the year of sample collection. The value of A_(450 nm) for NS1 was from high to low in 2017, 2018, 2015 and 2016. The value of A_(450 nm) for IgM was from high to low in 2018, 2016, 2017 and 2015. The value of A_(450 nm) for IgG was from high to low in 2015, 2016, 2017 and 2018. Conclusions Thereare prevalence of dengue from 2015 to 2018 after epidemic in Nanning. The passive immunity in population that obtained owing to dengue epidemic gradually drops as time goes on. Susceptibility of population is markedly increased from 2018.
