PPH联合微切口外痔剥除术在重度混合痔中的应用价值

摘要
目的研究吻合器痔上黏膜环切术(PPH)联合微切口外痔剥除术用于重度混合痔的临床价值。方法将90例重度混合痔患者随机分为观察组与对照组,各45例。对照组实施传统外剥内扎术,观察组实施PPH联合微切口外痔剥除术。对比两组的治疗效果。结果观察组的手术时间、出血量、创面愈合时间及住院时间均显著优于对照组(P<0.05)。两组术后VAS评分呈下降趋势,组内不同时间点对比差异显著(P<0.05);观察组术后不同时间点VAS评分均显著低于对照组(P<0.05)。观察组的术后并发症总发生率显著低于对照组(P<0.05)。结论 PPH联合微切口外痔剥除术治疗重度混合痔,可减轻患者术后疼痛,且并发症发生率低,利于患者术后恢复。
Objective To study the clinical value of PPH combined with micro-incision external hemorrhoidectomy in severe mixed hemorrhoids. Methods Ninety patients with severe mixed hemorrhoids in Hanjiang Hospital Affiliated to Xi'an Medical University were randomly divided into observation group and control group, with 45 cases in each group. The control group received traditional exfoliation and internal ligation, while the observation group received PPH combined with micro-incision external hemorrhoidectomy. The therapeutic effects of the two groups were compared. Results The operation time, bleeding volume, wound healing time and hospitalization time of the observation group were significantly better than those of the control group(P<0.05). The VAS scores of the two groups showed a downward trend, and there were significant differences in VAS scores of each group at different time points(P <0.05). The VAS scores of the observation group at different time points after operation were significantly lower than those of the control group(P<0.05). The total incidence of postoperative complications in the observation group was significantly lower than that in the control group(P <0.05).Conclusion PPH combined with micro-incision hemorrhoidectomy for severe mixed hemorrhoids can relieve postoperative pain, with low incidence of complications, which is conducive to the postoperative recovery of patients.
