品管圈在提高口腔肿瘤皮瓣修复术后口腔舒适度中的应用研究

摘要

目的探讨"品管圈"在提高口腔肿瘤皮瓣修复术后口腔舒适度的应用效果。方法组建活动小组,确立以"提高口腔肿瘤皮瓣修复术后患者口腔舒适度"为活动主题,进行现状调查,设立目标,从人、物、法3个方面查找原因,经过真因分析,确定要因,制定并采用红黄绿家庭参与型口腔护理模式。选择2017年01月～2017年12月口腔肿瘤术后患者34例为活动前组,2018年01月～2018年11月口腔肿瘤术后患者31例为活动后组。活动前组采用传统的口腔护理方法对患者行口腔护理,活动后组采用红黄绿家庭参与型口腔护理模式对患者进行口腔护理。活动前后均采用线性标尺视觉模拟评分法对口腔肿瘤皮瓣修复患者术后口腔舒适度进行调查。结果开展品管圈活动2年后,口腔皮瓣修复患者术后口腔舒适度活动后明显高于活动前(P<0.0001),护士口腔护理理论考核与技能操作成绩高于活动前(P<0.01),患者家庭口腔照顾参与度高于活动前(P<0.0001),出院时患者自护能力高于活动前(P<0.0001)。结论品管圈改善了患者术后口腔不舒适症状,增强其康复信心,提高了护士护理技能水平,体现了家庭照护及自护能力在患者康复中的重要性。
        Objective To evaluate the effect of QCC in improving the comfortability of patients with oral cancer and undergo skin flap reconstruction.Methods Quality management circle activity group was established to improve the postoperative comfortability of patients with oral cancer underwent flap reconstruction.Theme activities were carried out and the primary goal was set up.The primary elements were analyzed from the following 3 categories:human source,contents, and methodologies.We initiated a red/yellow/green oral nursing mode and applied this mode to patients with oral cancer and had flap reconstruction from February to December 2017.Patients who had routine oral nursing during February to November 2018 served as control.Visual analogue scale was applied to evaluate the comfortability of patients 1 month after operation.Results After 2 years follow-up,patients who had QCC management had significant improved comfortability(P<0.01), family engagement(P<0.01),and self-care abilities(P<0.01).than those who had routine oral nursing.Conclusion QCC management improved the comfortability and self-confidence of patients,increased the nursing care ability, and emphasized the importance of family care in the nursing activity.
