阴道彩色多普勒超声检测宫颈癌血流动力学改变的价值

摘要
目的探讨阴道彩色多普勒超声检测宫颈癌血流动力学改变的价值。方法随机选取2015年5月至2017年5月保定市第二中心医院诊治的30例宫颈癌患者作为研究对象,将其设为宫颈癌组,同时选取同期保定市第二中心医院接收的健康体检的30例女性作为对照组。对两组研究对象进行阴道彩色多普勒超声检查,然后对两组研究对象的超声声像图特征、血流信号分布、血流动力学参数、宫颈癌组不同临床分期、组织学分型患者的血流动力学参数进行统计分析。结果宫颈癌组患者的血流信号0级、+级比例均显著高于对照组,差异具有统计学意义(P<0.05),++级、+++级比例均显著低于对照组,差异具有统计学意义(P<0.05),血流动力学PSV显著高于对照组,差异具有统计学意义(P<0.05),RI显著低于对照组,差异具有统计学意义(P<0.05),血流动力学PSV随着临床分期的提升而提升,差异具有统计学意义(P<0.05),RI随着临床分期的下降而下降,差异具有统计学意义(P<0.05);腺癌患者的血流动力学PSV显著高于鳞癌,差异具有统计学意义(P<0.05),RI显著低于鳞癌,差异具有统计学意义(P<0.05)。结论阴道彩色多普勒超声检测宫颈癌血流动力学改变的价值高。
Objective To investigate the value of transvaginal color Doppler ultrasound in the detection of hemodynamic changes in cervical cancer.Methods 30 patients with cervical cancer from May 2015 to May 2017 in Baoding the Second Central Hospital were randomly selected and regarded as cervical cancer group, and 30 healthy women in our hospital during the same time were selected as control group. Patients in the two groups were given transvaginal color Doppler ultrasound examination. The sonographic characteristics, blood flow signals distributions, hemodynamic parameters of the two groups and the hemodynamic parameters of patients in different clinical stages, histological types in the cervical cancer group were statistically analyzed. Results Theratios of blood flow signal grade 0 and grade + in the cervical cancer group were significantly higher and the ratios of grade + + and grade + + + were significantly lower than those in the control group, all with statistically significant differences(P<0.05). The hemodynamics PSV was significantly higher and the RI was significantly lower in the cervical cancer group than that in the control group, all with statistically significant differences(P<0.05).The hemodynamics PSV increased with the increase of clinical stages and the RI decreased with the decline of clinical stages, all with statistically significant differences(P<0.05). The hemodynamics PSV of patients with adenocarcinoma was significantly higher(P<0.05) and the RI was significantly lower than that of patients with squamous cell carcinoma, all with statistically significant differences(P<0.05).Conclusions Transvaginal color Doppler ultrasound is of high value in the detection of hemodynamic changes in cervical cancer.
