沟通障碍的痔疮患者清洁灌肠插管方法的探讨

摘要
目的探讨沟通障碍的痔疮患者清洁灌肠插管方法。方法选择2014年6月~2015年10月本地区讲土方言导致沟通障碍的痔疮需清洁灌肠的患者60例,随机分成对照组和实验组,每组各30例,对照组使用常规方法插管清洁灌肠;实验组采用平板电脑宣教联合手法诱导肛门括约肌松弛,再充分润滑肛门、肛管直肠插管清洁灌肠,比较两组患者的肛管一次性插管成功率、肛门直肠疼痛程度及出血发生率,发放调查问卷调查满意度。结果实验组的肛管一次性插管成功率及满意度高,肛门直肠疼痛程度及出血发生率均低于对照组,两组比较差异有统计学意义(P<0.05)。结论在沟通障碍的痔疮需要灌肠患者中,采用平板电脑宣教联合手法诱导肛门括约肌松弛,再充分润滑肛门、肛管直肠插管清洁灌肠,能明显提高患者满意度及肛管一次性插管成功率,减轻疼痛的程度及降低出血的发生率,值得临床推广。
Objective To explore the method of cleaning enema with intubation for hemorrhoids patients with communication barrier. Methods 60 local hemorrhoids patients with communication barrier caused by dialect who need cleaning enema from June 2014 to October 2015 were randomly divided into control group and experimental group, 30 cases in each group. The control group received traditional cleaning enema with intubation. The experimental group received tablet computer education combined with manipulation for inducing the relaxation of anal sphincter, the sufficient lubrication of the anus and the anal canal, then the cleaning enema with rectal intubation. Success rate of first intubation, pain intensity of anus and rectum, and the incidence of bleeding were compared between two groups. Questionnaires were used to survey the patients' satisfaction. Results In the experimental group, the success rate of first intubation and the patients' satisfaction were higher, while the pain intensity of anus and rectum, the incidence of bleeding were lower than those of the control group, the difference between the two groups was statistically significant(P < 0.05). Conclusion In the hemorrhoids patients with communication barrier who need cleaning enema, the employment of tablet computer education combined with manipulation for inducing the relaxation of anal sphincter, the sufficient lubrication of the anus and the anal canal, then the cleaning enema with rectal intubation can significantly improve patients' satisfaction and the success rate of first intubation, reduce the pain intensity and the incidence of bleeding. It is worthy to be clinical popularized.
