河北省沧州市2011-2017年布鲁氏菌病血清学检测结果分析

摘要
目的了解沧州市人间布鲁氏菌病(简称布病)的感染状况,掌握该市布病流行趋势,为制订防控策略提供科学依据。方法在全市18个县(市、区)开展高危人群的血清学检测。按中华人民共和国卫生行业标准布鲁氏菌病诊断标准(WS 269-2007)确诊布病病例。结果沧州市2011-2017年全市共调查布病高危人群57 616人,血清学检测9 075人,平均阳性率为5. 34%。其中2013年最高,为8. 67%; 2011年最低,为3. 96%,差异有统计学意义(χ~2=39. 53,P <0. 01)。地区间阳性率不同,南皮县最高,为13. 08%;海兴县最低,为0. 49%,各县(市、区)阳性率差异有统计学意义(χ~2=230. 87,P <0. 01)。不同职业人群阳性率比较,放牧、屠宰、饲养较高,皮毛及乳肉贺加工、贩运人员最低,不同职业间差异有统计学意义(χ2=28. 56,P <0. 01)。结论沧州市布病疫情较严重,要继续加强人间布病监测,加大高危人群健康干预力度,积极与畜牧部门合作,加强源头防治,降低全市人群布病的感染率。
        Objective To understand the infection status of brucellosis( abbreviated as brucellosis) in Cangzhou City,and master the epidemic trend of brucellosis in the city,so as to provide scientific basis for formulating prevention and control strategies. Methods Serological testing of high-risk populations was carried out in 18 counties( cities,districts) throughout the city. Cases of brucellosis were confirmed according to the diagnostic criteria for brucellosis in the health industry of the People's Republic of China( WS 269-2007). Results From 2011 to 2017,a total of 57 616 people with high-risk brucellosis were investigated in cangzhou,and 9 075 people were tested by serology,with an average positive rate of 5. 34%. The highest rate was 8. 67% in 2013. The lowest rate was 3. 96% in 2011,with statistically significant difference( χ~2= 39. 53,P < 0. 01). The positive rate was the highest in Nanpi County( 13. 08%). Haixing County had the lowest positive rate( 0. 49%),and the positive rate was statistically significant( χ~2= 230. 87,P < 0. 01). The positive rates of different occupations were higher in grazing,slaughtering and feeding,and the lowest in fur,milk and meat processing and trafficking. The difference was statistically significant among different occupations( χ2= 28. 56,P < 0. 01). Conclusion Cangzhou has a serious epidemic of brucellosis,so it is necessary to continue to strengthen brucellosis monitoring,increase the health intervention of high-risk groups,actively cooperate with animal husbandry departments,strengthen the prevention and treatment from headstream,and reduce the infection rate of brucellosis in the whole city.
