摘要
目的:探讨个体化医疗理念在专业型研究生神经外科临床教学中的价值。方法:纳入专业型研究生共60人,随机分为个体化理念教学组(30人)和常规教学组(30人),前者采用个体化医疗理念教学,后者采用常规教学。采用问卷及考核的方式加以评估,用t检验及卡方检验作统计学分析。结果:课程满意度问卷调查显示91.67%(55/60)的学生认为个体化原则下的课程组织形式更合理,85.00%(51/60)的学生认为相比传统教学方法更好。课程效果问卷调查显示实验组中93.33%(28/30)的学生认为能提高学习兴趣、90.00%(27/30)的学生认为能拓宽诊治思路、93.33%(28/30)的学生认为能促进临床思维能力、90.00(27/30)%的学生认为能优化疾病临床诊疗流程、93.33%(28/30)的学生认为能增进医患沟通能力,相比对照组的相应指标,差异均有统计学意义(P<0.05)。实验组考核成绩为85.76±7.43分,对照组为74.82±7.91分,两组存在显著差异(P=0.0000)。结论:个体化医疗理念下的神经外科实习教学模式有助于专业型研究生全面掌握临床信息,制定更科学、更具个体化的治疗方案,并有助于培养其临床思维能力。
        Objective To explore the value of personalized medicine in the clinical teaching of Neurosurgery among clinical-type postgraduates. Methods Totally 60 clinical-type postgraduate were divided into experimental class and control class randomly,with 30 students in each group. The experimental class received personalized medical teaching and the control class received conventional teaching. The effect of teaching was evaluated by questionnaire survey and examination. The data were analyzed through t-test and Chi-square test. Results Questionnaire survey of satisfaction showed that 91. 67%( 55/60) students thought personalized teaching more rational,and 85. 00%( 51/60) students believed new course better than the conventional one. Questionnaire survey of teaching effect showed that in experimental class 90. 00%( 27/30) students believed improved diagnosis and treatment,93. 33%( 28/30) students felt increased learning interest,90. 00%( 27/30) students felt their medical view expanded,93. 33%( 28/30) students had consolidated clinical thinking,and 93. 33%( 28/30) students improved negotiation with patient,all issues were significantly better than control class( P < 0. 05). Meanwhile,statistical difference was found in average score between experiment class and control class( 85. 76 ± 7. 43 vs. 74. 82 ± 7. 91)( P = 0. 0000). Conclusion Personalized medicine is better for clinical-type postgraduates to make personalized diagnosis and treatment plan,and to cultivate their clinical thinking and the concept of evidence based medicine during clinical practice of neurosurgery.
