摘要
目的:通过在军队医院进行慢性病实时监测系统研发和应用,以解决军队医院慢性病管理中临床医生工作繁忙、上报依从性差、漏报率高等问题,从而实现军队医院对慢性病就诊住院患者的自动筛查、统计与上报功能。方法:我院于2015年以军队医院信息系统为基础,采用Client/Server体系结构,基于Oracle数据库,与现有的医院信息系统(hospital information system,HIS)、实验室信息系统(laboratory information system,LIS)以及检查系统实现无缝连接,研发了慢性病实时监测系统,并于2015~2018年将该系统运用于本院慢性病监测。通过比较该系统运用前后我院慢性病上报情况,评价其临床应用效果。该系统以疾病诊断为核心信息采集"触发点",以实时运行为采集方式,根据医生相关诊断,就能无遗漏地自动采集住院、门诊慢性病患者相关信息,及时上报至各级疾病预防控制中心(center for disease control and prevention,CDC),从而实现对慢性病的统一监测和管理。结果:通过采用慢性病实时监测系统,对我院慢性病患者就诊信息实现自动采集、自动检索,提高了慢性病患者信息采集的准确性和及时性。结论:通过慢性病实时监测系统的应用,实现了门诊、住院慢性病患者信息的准确、全面、无遗漏、全覆盖及时上报,可为地方和国家提供可靠数据,便于开展随访和健康指导。
        Objective:To investigate the development and application of real-time chronic disease surveillance system in military hospital,in order to solves many problems in chronic disease management,such as clinician′s busy work,low compliance and high missing rate of report,and thus realizes the functions including automatic screening,counting and reporting of patient with chronic disease in military hospital.Methods:The system was based on the military hospital information system,set up in a Client/Server architecture,based on Oracle database,and interface with the existing hospital information system(HIS),laboratory information system(LIS) was researched and developted real-time chronic diseaes,and it was applied to chronic disease monitoring in our hospital from 2015 to 2018.clinical application effect was evaluated by comparison of the report of chronic disease before and after application. It taked the disease diagnosis as the core information collection "trigger point",and the real-time operation as collection method. According to the doctor′s relevant diagnosis,the relevant information of patients with chronic diseases in inpatients and outpatients can be automatically collected and reported to center for disease control and prevention(CDC) in time to achieve unified surveillance and management of chronic diseases.Results:After the real-time chronic disease surveillance system was applied in our hospital,the automatic collection and retrieval of chronic disease patients′ medical information improved the accuracy and timeliness of information collection.Conclusion:The application of real-time chronic disease surveillance system in military hospital guarantees the information of patients with chronic diseases in inpatients and outpatients can be reported to CDC accurately,comprehensively,without missing and full coverage,provides reliable data and facilitates follow-up and health guidance.
