摘要
目的:分析全自动生化分析仪总蛋白(TP)测定对钙(Ca~(2+))测定产生干扰的原因,并进一步提出消除措施。方法:使用日立LABOSPECT 008全自动生化分析仪连续测定同一混合血清标本Ca~(2+)水平8次,测定结果 R的均值作为参照值。测定TP之后,使用另外的比色杯测定Ca~(2+)水平,结果为T1;不更换比色杯,常规清洗程序后测定Ca~(2+)水平,结果为T2;不更换比色杯,多次清洗后测定Ca~(2+)水平,结果为T3。测试结果与参照值相差5%及以上认为存在携带污染。采用SPSS 19.0软件进行数据处理及统计分析。结果:T1均值与参照值偏差小于5%(P=0.644),T2均值与参照值偏差达到16.042%(P=0.000),T3均值与参照值偏差小于5%(P=0.691)。结论:在TP测定后测定Ca~(2+),可忽略试剂针携带污染对测量的影响,但比色杯携带污染的影响不可忽略。干扰原因有可能是TP测定试剂量相对较大,清洗不彻底导致2种试剂发生螯合反应。因此在测定Ca~(2+)时,应避免使用TP测定后的比色杯或将比色杯彻底清洗,可有效消除干扰。
        Objective To analyze the interference of total protein(TP) determination on calcium(Ca~(2+)) determination by automatic biochemical analyzer, and further put forward the elimination measures. Methods The Ca~(2+)level of the same mixed serum sample was continuously measured by Hitachi LABOSPECT 008 automatic biochemistry analyzer, and the average R value was used as the reference value. After the determination of TP, the other colorimetric cup was used to determine the Ca~(2+)level, and the result was T1. Without replacing the colorimetric cup, the Ca~(2+)level was measured after the routine cleaning procedure, and the result was T2. The colorimetric cup was not replaced and the Ca~(2+)level was measured after repeated cleaning, then the result was T3. Contamination was considered to be existent in case the difference was not lower than 5%between the measured values and the reference ones. SPSS 19.0 software was used for data processing and statistical analysis.Results The deviation between T1 mean and reference value was less than 5% and t test P was 0.644. The deviation between T2 mean and reference value was 16.042%, and t test P was 0.000. The deviation between T3 mean and reference value was less than 5% and t test P was 0.691. Conclusion The reagent has the contamination ignorable to the Ca~(2+)determination after TP measurement, while the cuvette contamination can not be ignored. It's suggested the cause of cuvette contamination may be the chelation occurring between the residual TP reagent and Ca~(2+). During the determination of Ca~(2+), the interference can be effectively eliminated by abandoning the cuvette involved in TP determination or cleaning it completely.
