产科护士支持纯母乳喂养信念的现象学研究

摘要
目的深入了解产科护士支持纯母乳喂养的信念情况。方法 2018年6月,目的抽样法选取某三级甲等妇产专科医院的产科护士10名为研究对象。采用现象学研究方法对其进行半结构式访谈,Colaizzi 7步分析法进行资料分析。结果共有两个主题,分别为医院纯母乳喂养的总体效果理想但仍需加强和产科护士支持纯母乳喂养信念不足的因素众多;在"产科护士支持纯母乳喂养信念不足的因素众多"主题下提炼了知识和技能不足、支持纯母乳喂养的能力和信心不足、安全和满意度同支持纯母乳喂养之间的矛盾、社会和团队支持不足及资源不足等亚主题。结论产科护士在支持纯母乳喂养中面临众多的阻碍,需要重视教育培训,研制乳汁量评价工具,实施科学管理并建立支持性环境。
Objective To explore the beliefs of obstetric nurses in supporting exclusive breastfeeding.Methods By adopting the purposive sampling method,10 obstetric nurses from a obstetrics and gynecology hospital were selected in June 2018.Semi-structured interviews were conducted using phenomenological research methods,and Colaizzi's 7-step analysis method was used for data analysis.Results Two themes were extracted,including "The practice of exclusive breastfeeding in hospitals are generally ideal but still need to be strengthened"and "There are some obstacles to setting up a strong belief of supporting exclusive breastfeeding".There were also several sub-themes:insufficient knowledge and skills,perceived lack of ability and confidence,contradiction between safety,satisfaction and exclusive breastfeeding,poor team and social support and insufficient resources.Conclusions To improve the belief of obstetric nurses in supporting exclusive breastfeeding,attention should be paid to education and training,the development of milk volume assessment tools,the implementation of scientific management,and the establishment of supportive environment.
