超声联合CT或MRI在诊断冠状动脉疾病中的应用比较

摘要
目的比较超声联合CT或MRI在诊断冠状动脉疾病中的应用价值。方法选取我院2015年8月至2017年8月69例临床怀疑冠状动脉疾病患者为研究对象,均在我院接受超声心动图、冠状动脉CTA、MRI检查及冠状动脉造影术。以冠状动脉造影结果为金标准,计算超声联合冠状动脉CTA或MRI诊断冠状动脉疾病的灵敏度、特异度、阳性预测值、阴性预测值、总符合率。结果共69例疑似冠状动脉疾病患者经冠状动脉造影术证实,管腔狭窄≥50%的冠状动脉共179支,管腔狭窄<50%的冠状动脉共97支。超声联合冠状动脉CTA诊断冠状动脉疾病的灵敏度、特异度、阳性预测值、阴性预测值、总符合率、Kappa值分别为89.4%、92.8%、95.8%、82.6%、90.6%、0.80。超声联合MRI诊断冠状动脉疾病的灵敏度、特异度、阳性预测值、阴性预测值、总符合率、Kappa值分别为92.7%、89.7%、94.3%、87.0%、91.7%、0.82。两种检查方案诊断冠状动脉疾病的灵敏度、特异度、阳性预测值、阴性预测值、总符合率比较差异无统计学意义(P>0.05)。结论超声联合冠状动脉CTA或MRI均可准确反映冠状动脉狭窄情况,为冠状动脉疾病的诊断提供影像学依据,可根据患者个人意愿与经济状况选择不同检查方法。
        Objective To compare the value of ultrasound combined with coronary CT angiography or MRI in the diagnosis of coronary artery disease(CAD). Methods A total of 69 patients with suspected CAD who were admitted to the hospital from August 2015 to August 2017 were selected as subjects. All of them underwent echocardiography, coronary CT angiography, MRI examination and coronary angiography in the hospital. The sensitivity, specificity, positive predictive value, negative predictive value and total coincidence rate of ultrasound combined with coronary CT angiography or MRI in diagnosing CAD were calculated with results of coronary angiography as the golden standard. Results Coronaryangiography confirmed that there were a total of 179 coronary arteries with luminal stenosis of 50% or higher and 97 coronary arteries with luminal stenosis lower than 50%.The sensitivity, specificity, positive predictive value, negative predictive value, total coincidence rate and Kappa value of ultrasound combined with CT in diagnosing CAD were 89.4%, 92.8%, 95.8%, 82.6%, 90.6% and 0.80, respectively, while those of ultrasound combined with MRI were 92.7%, 89.7%, 94.3%, 87%, 91.7% and 0.82, respectively, without significant difference in the sensitivity, specificity, positive predictive value, negative predictive value or total coincidence rate between the two diagnostic schemes(P>0.05). Conclusion Ultrasound combined with coronary CT angiography or MRI can reflect the situation of coronary artery stenosis and provide imaging evidence for the diagnosis of CAD accurately. Different examination methods can be selected according to patient's intention and economic status.
