罕见完全内脏转位合并脾动脉瘤、胰头占位1例

摘要
<正>病例女,56岁。近1年来反复腹痛,体质量下降约12.5 kg,来我院检查。自述腹痛发作时大致位于左上腹,无放射痛,无腹胀、腹泻及黄疸。门诊查体:身高161 cm,体质量49 kg,血压138/86 mmHg (1 mmHg=0.133 kPa),腹部未触及明显包块,未见明显压痛区,心尖搏动位于右锁骨中线第5肋间内侧。腹部B超示:腹腔内脏转位(Situs inversus,SI);脾门处见大小约3.9 cm×3.8 cm囊性回声,内可见动脉血流信号(图1);胰腺探查不清。提示:脾门处囊性肿物(考虑脾动脉瘤),腹腔SI。进一步行上下腹部CT平扫+增强示:腹腔SI;胰头部团
