老年骨质疏松髋部骨折围术期骨转换指标及术后隐性失血的影响

摘要
目的探讨老年骨质疏松髋部骨折围术期骨转换指标及术后隐性失血的影响。方法选择2017年1月至12月本院收治的老年髋部骨折行手术治疗者80例,按照是否合并骨质疏松分为两组,各40例,观察组为合并骨质疏松,对照组未合并骨质疏松者,比较术后两组骨密度及骨降钙素水平,两组骨转化相关指标,两组手术前后血红蛋白变化及两组术后引流时间及量的变化。结果术后,观察组骨密度水平及骨降钙素水平均低于对照组(P<0.05),观察组碱性磷酸酶水平高于对照组(P<0.05),血清Ⅰ型胶原交联羧基末端肽水低于对照组(P<0.05),术后观察组血红蛋白水平低于术前及术后对照组(P<0.05),观察组留置引流管时间长于对照组(P<0.05),总引流量大于对照组(P<0.05)。结论针对老年骨质疏松合并髋关节手术者,有效的针对骨质疏松进行干预,提高骨密度水平,调节骨转化指标,对于减少术后隐性失血有重要意义。
Objective To investigate the perioperative bone turnover index and the effect of postoperative recessive blood loss in elderly osteoporotic hip fractures.Methods 80 cases undergoing surgical treatment for hip fractures treated in our hospital from January 2017 to December 2017 were divided into two groups according to whether they had osteoporosis,40 cases in each group,and observation group(with osteoporosis).In the control group(without osteoporosis),bone mineral density and bone calcitonin levels in the two groups were compared,bone turnover related parameters in the two groups,changes in hemoglobin before and after the surgery in both groups,and changes in drainage time and volume after surgery in the two groups.Results Postoperatively,the bone mineral density and bone calcitonin levels in the observation group were lower than those in the control group(P<0.05).The alkaline phosphatase level in the observation group was higher than that in the control group(P<0.05),and the serum type I collagen was cross-linked in the car boxyl group.The water content of terminal peptide was lower than that of the control group(P <0.05).The hemoglobin level in the observation group was lower than that in the preoperative and postoperative control group(P<0.05).The indwelling drainage tube in the observation group was longer than the control group(P<0.05).The drainage volume was greater than that of the control group(P<0.05).Conclusion For elderly osteoporosis patients with hip joint surgery,effective intervention for osteoporosis,increase of bone mineral density and adjustment of bone turnover index are important for reducing postoperative recessive blood loss.
