医院船“和谐使命-2018”海外超声医疗服务的实践与思考

摘要
目的总结"和谐使命-2018"医院船海外超声医疗服务的工作,为今后医院船海外医疗服务提供借鉴。方法回顾"和谐使命-2018"任务南太平洋地区巴布亚新几内亚、瓦努阿图、斐济和汤加4国和中南美洲地区委内瑞拉、格林纳达、多米尼克、安提瓜和巴布达、多米尼加、厄瓜多尔6国的超声医疗服务工作完成情况。结果本任务共计完成各类超声检查5 277人次,其中为我驻外使馆人员和国外军政要员及家属体检614人次,各国民众检查4 663人次。医院船主平台完成检查5 170人次(含体检614人次、住院19人次及门诊4 639人次),前出巡诊102人次(含直升机赴斐济瓦图莱莱岛巡诊10人次,小艇赴格林纳达卡里亚库岛巡诊58人次),便携式超声急救床旁检查5人次,中-巴新联合应急医学救援演练1次。所有超声检查均顺利完成,获得了积极的国际影响。分析了任务期间医院船超声医疗工作的特点,即远航时间长,患者需求多,社会期望高,地区差异大等特点。结论为保障"和谐使命-2018"任务顺利完成,超声工作包括但不局限于制定因时、因地、因需的超声工作规范与预案,合理配备超声医生,确保检查快速、有效、安全,重视超声质量管理,做好交流沟通等环节,对今后持续改进医院船海外超声医疗服务工作具有一定的参考价值。
Objective To sum up the work experience of ultrasonography during overseas medical service of "Mission Harmony-2018", so as to provide valuable evidence for future overseas medical service onboard the hospital ship.Methods The paper made a retrospective analysis and summary of ultrasonography during overseas medical service in such island countries as Papua New Guinea, Vanuatu, Fiji and Tonga in the South Pacific Region, and in Latin American countries, including Venezuela, Grenada, Dominica, Antigua and Barbuda, Dominica, and Ecuador.Results During overseas medical service, a total of 5 277 man times of ultrasonographic examinations were performed, including 614 man times of health check for our embassy staffs and their dependents and VIPs of related countries, and 4 663 local residents. Completed on the hospital ship main platform were 5 170 man times of ultrasonographic detections, including 614 man times of physical examination, 19 man times of ultrasonographic detections for inpatients and 4 639 man times of detections for outpatients. One hundred and two man times of patient visits(including 10 man times of patient visits to Vatulele Island, Fiji, by helicopter, and 58 man times of patient visits to Carriacou Island, Grenada by small craft), and 5 man times of bed-side ultrasonographic detections by using portable ultrasound detectors were also performed successfully during the overseas medical service. Besides, a Chinese and Papua New Guinea joint emergency medical service exercise was also conducted. All the ultrasonographic examinations were successfully completed, and positive global scale impacts were thus obtained. At the same time, the paper analyzed the main features of ultrasonography onboard the hospital ship during the mission, i.e. the features of prolonged navigation, multiple demands from patients, high expectations from society, varied and great differences between regions, etc.Conclusion To ensure smooth completion of "Mission Harmony-2018", the working scope of ultrasonography might include, but is not just limited to the development of the working specifications and profiles in accordance with the time and region, as well as requirements of patients, the proper staffing of ultrasonographic technicians, so as to assure rapidity, efficiency and safety of ultrasonographic detections. Furthermore, attention should be paid to the management on ultrasonic quality, interpersonal communication and summary of working experience. To sum up, our experience onboard the hospital ship is of value to further improvement in future overseas medical service.
