摘要
目的分析新疆地区人群血清尿酸水平与糖脂代谢生化指标的相关性,研究高尿酸血症的危险因素,为本地区人群更全面的应对高尿酸血症提供进一步的研究依据。方法 2017年3月至2018年5月选取高尿酸血症患者1 519例(高尿酸血症组)和同期体检健康者1 808例(对照组)为研究对象,采用标准方法进行体格检查和血清生化指标检测,并采用SPSS21.0软件进行统计分析。结果高尿酸血症组血尿酸水平为(463.08±69.10)μmol/L,对照组血尿酸水平为(265.20±78.20)μmol/L,高尿酸血症组腰围、臀围、腰臀比(WHR)、体质量指数(BMI)、收缩压(SBP)、舒张压(DBP)、血尿酸、三酰甘油(TG)和低密度脂蛋白胆固醇(LDL-C)水平均高于对照组,高密度脂蛋白胆固醇(HDL-C)低于对照组,差异有统计学意义(P<0.05);男性和女性高尿酸血症组中腰围、BMI、SBP、DBP、血尿酸、TG均高于对照组,差异有统计学意义(P<0.05);空腹血糖(FBG)和LDL-C在男性的两组间比较差异有统计学意义(P<0.05),臀围、WHR、总胆固醇(TC)、HDL-C在女性的两组间比较差异有统计学意义(P<0.05);不同性别人群尿酸水平随年龄的增长而增加,男性的尿酸水平要高于女性,差异有统计学意义(P<0.05);不同尿酸水平下糖脂代谢生化指标比较,男性人群各指标在组间差异有统计学意义(P<0.05),其中TG随着尿酸水平的升高而升高,女性人群中除FBG在各组间比较差异无统计学意义外(P>0.05),其余指标在各组间差异均有统计学意义(P<0.05),HDL-C随尿酸水平的升高逐渐降低;不同性别人群血尿酸与TG、LDL-C存在正相关,与HDL-C呈负相关(P<0.05),与FBG未出现相关性;肥胖、高血压、高三酰甘油血症、高胆固醇血症在高尿酸血症组和对照组中的分布差异有统计学意义(P<0.05);糖脂代谢相关疾病的检出率,高尿酸血症组均高于对照组,高血糖在两组的分布差异无统计学意义(P>0.05)。对高尿酸血症进行多因素Logistic回归分析,结果显示肥胖(OR=1.241,95%CI=1.024~1.504)、高血压(OR=2.147,95%CI=1.758~2.623)、高三酰甘油血症(OR=2.269,95%CI=1.912~2.693)均为高尿酸血症的危险因素。结论新疆地区人群尿酸与血脂代谢生化指标存在相关性,高尿酸血症与肥胖、高血压、高三酰甘油血症、高胆固醇血症密切相关。
        Objective To analyze the correlation between serum uric acid level and biochemical markers of glucose and lipid metabolism in Xinjiang population,and to study the risk factors of hyperuricemia,provide further research basis for the more comprehensive response to hyperuricemia in the region.Methods From March 2017 to May 2018,1 519 patients with hyperuricemia(hyperuricemia group)and 1 808 healthy controls(control group)were selected as subjects.Physical examination and serum biochemical indexes were detected by standard method,and statistical analysis was made by spss21.0 software.Results The level of serum uric acid(SUA)in hyperuricemia group was(463.08±69.10)μmol/L,the SUA level in control group was(265.20±78.20)μmol/L.The levels of waist circumference,hip circumference,waist hip ratio(WHR),BMI,systolic pressure(SBP),diastolic pressure(DBP),SUA,triglyceride(TG)and low density lipoprotein cholesterol(LDL-C)in hyperuricemia group were higher than those in control group,high density lipoprotein cholesterol(HDL-C)was significantly lower than that in control group,the difference was statistically significant(P<0.05).The waist circumference and BMI,SBP,DBP,SUA,TG in male and female hyperuricemia group were higher than those in control group,the difference was statistically significant(P<0.05).There was a significant difference in fasting blood-glucose(FBG)and LDL-C between the two groups in men(P<0.05),the hip circumference,waist-to-hip ratio and total cholesterol(TC),HDL-C were significantly different between the two groups in women(P<0.05).The serum uric acid level of different gender groups increased with age,and the serum uric acid level of males was higher than that of females,the difference was statistically significant(P<0.05).Compared with the biochemical indexes of glycolipid metabolism in different uric acid levels,there were statistical differences among different uric acid groups in male population,and TG increased with the increase of uric acid level.In the female population,there was no significant difference in FBG in different uric acid groups(P>0.05),but there were significant differences in the other indexes in each group(P<0.05).The HDL-C decreased gradually with the increase of uric acid level,the difference was statistically significant(P<0.05).There was a positive correlation between SUA and TG and LDL-C in different gender groups,and a negative correlation with HDL-C(P<0.05),and no correlation with FBG.The distribution of obesity,hypertension,hypertriglyceridemia and hypercholesterolemia in hyperuricemia group and healthy control group was significantly different(P<0.05).The detection rate of disease in hyperuricemia group was higher than that in healthy control group.There was no significant difference in the distribution of hyperglycemia between the two groups(P>0.05).Multivariate logistic regression analysis of hyperuricemia showed obesity(OR=1.241,95%CI=1.024-1.504),hypertension(OR=2.147,95%CI=1.758-2.623),high triglyceride blood Symptoms(OR=2.269,95%CI=1.912-2.693)are risk factors for hyperuricemia.ConclusionThere is a correlation between serum uric acid and blood lipid metabolism in Xinjiang population.Hyperuricemia is closely related to obesity,hypertriglyceridemia and hypercholesterolemia.
