肌力、脂肪含量与绝经后骨质疏松症患者骨密度T值的相关性分析

摘要
目的探讨肌力、脂肪含量与绝经后骨质疏松症患者骨密度T值的相关性,为临床防治绝经后骨质疏松症提供依据。方法选择2015年6月-2017年6月江苏省中医院骨伤科收治的骨质疏松症患者84例设为观察组,根据骨密度T值测定结果分为骨量减少组32例、骨质疏松组41例及严重骨质疏松组11例;另选择同期入院健康体检者40例设为对照组。采用Inbody720型电子背力计分别测定各组腰背部伸肌力(取平均值),利用美国Hologic公司生产的全身型双能X线骨密度仪测定各组全身脂肪及腰椎L1~4骨密度值,采用SPSS Pearson相关性分析软件对肌力、脂肪含量对绝经后骨质疏松症患者骨密度T值的相关性进行分析。结果观察组绝经后骨质疏松症患者肌力、骨密度水平低于对照组,腹部脂肪含量高于对照组,差异均有统计学意义(P<0. 05)。骨量减少组肌力、骨密度水平高于骨质疏松组和严重骨质疏松组,脂肪含量低于骨质疏松组和严重骨质疏松组,差异均有统计学意义(P<0. 05);骨质疏松组肌力、骨密度水平高于严重骨质疏松组,脂肪含量低于严重骨质疏松组,差异均有统计学意义(P<0. 05)。绝经后骨质疏松症骨密度水平与肌力呈正相关性(P<0. 05),与脂肪含量呈负相关性(P<0. 05)。结论绝经后骨质疏松症患者肌力、脂肪含量较少,并且与骨密度之间存在紧密的联系。
        Objective To explore the correlation between muscle strength,fat content and bone mineral density( BMD) T value in patients with postmenopausal osteoporosis, provide a basis for clinical prevention and treatment of postmenopausal osteoporosis.Methods From June 2015 to June 2017,84 patients with postmenopausal osteoporosis treated in Department of Orthopedics,Jiangsu Provincial Hospital of Traditional Chinese Medicine were selected as observation group,according to the results of BMD T value,the patients were divided into osteopenia group( 32 patients),osteoporosis group( 41 patients),and severe osteoporosis group( 11 patients). Forty women receiving healthy physical examination in the hospital during the same period were selected as control group. Inbody720 electronic dynamometer was used to detect back body extensor strength. Whole-body dual-energy X-ray bone density meter was used to detect body fat and lumbar vertebrae L1-4 BMD. Pearson correlation analysis software was used to analyze the correlation between muscle strength,fat content and BMD T value in patients with postmenopausal osteoporosis. Results Muscle strength and BMD in observation group were statistically significantly lower than those in control group,while abdominal fat content in observation group was statistically significantly higher than that in control group( P<0. 05). Muscle strength and BMD in osteopenia group were statistically significantly higher than those in osteoporosis group and severe osteoporosis group,while abdominal fat content in osteopenia group was statistically significantly lower than those in osteoporosis group and severe osteoporosis group( P<0. 05). Muscle strength and BMD in osteoporosis group were statistically significantly higher than those in severe osteoporosis group,while abdominal fat content in osteoporosis group was statistically significantly lower than that in severe osteoporosis group( P<0. 05). BMD was positively correlated with muscle strength in patients with postmenopausal osteoporosis( P<0. 05),BMD was negatively correlated with fat content( P<0. 05). Conclusion Muscle strength and abdominal fat content in patients with postmenopausal osteoporosis are low,which are closely correlated with BMD.
