摘要
目的分析磁共振弥散加权成像(DWI)联合动脉自旋标记技术(ASL)对急性期脑缺血的诊断价值。方法对74例急性期脑缺血患者临床资料进行回顾性分析。发病24h均行常规MRI及DWI、ASL序列检查,观察DWI联合ASL对急性脑缺血的诊断价值。结果急性脑梗死患者患侧区ADC、rCBF值大于镜像区(P<0.05)。DWI、ASL对大面积脑梗死检出率均为100%。DWI对小面积脑梗死检出率大于ASL(P<0.05),ASL对短暂性脑缺血发作(TIA)检出率大于DWI(P<0.05)。结论 DWI、ASL在急性期脑缺血诊断中各有优劣,二者联合运用可显著提升对该疾病的检出率,对评估患者病情、指导临床治疗有重要作用。
        Objective To analyze the diagnostic value of diffusion-weighted imaging(DWI combined with arterial spin labeling(ASL) in the diagnosis of acute cerebral ischemia.Methods The clinical data of 74 patients with acute cerebral ischemia were retrospectively analyzed. They were given routine MRI and DWI and ASL sequence examinations at24 h after onset. The diagnostic value of DWI combined with ASL for acute cerebral ischemia was observed. Results The ADC and rCBF values in affected area of patients with acute cerebral infarction were larger than those in the mirror image area(P<0.05).The detection rates of large-area cerebral infarction by DWI and ASL both were 100%.The detection rate of small-area cerebral infarction by DWI was higher than that by ASL(P<0.05), and the detection rate of transient ischemic attack(TIA) by ASL was greater than that by DWI(P<0.05). Conclusion DWI and ASL have their advantages and disadvantages in the diagnosis of acute cerebral ischemia. The combination of the two can significantly improve the detection rate of disease, and it plays an important role in assessing the patient's condition and guiding clinical treatment.
