摘要
目的研究蛛网膜下腔出血大鼠血清中表皮生长因子受体(epidermal growth factor receptor,EGFR)、脂肪酸结合蛋白(fatty acid binding protein,FABP)、谷氨酸(glutamic acid,Glu)、天门冬氨酸(aspartic acid,Asp)的表达水平及意义。方法选取40只健康SD大鼠,雄性,随机分为对照组和模型组,每组20只。建立蛛网膜下腔出血大鼠模型。采用高效液相色谱法检测血清中EGFR、FABP、Glu、Asp水平,Western blot检测EGFR、FABP、Glu、Asp的表达,在光学镜下检测大鼠基底动脉的横截面积和血管壁厚度,分析大鼠血清中EGFR、FABP、Glu、Asp四项指标单独检测和联合检测的特异度、敏感度和准确度。结果模型组大鼠血清中EGFR、FABP、Glu、Asp水平显著高于对照组(P<0.05)。模型组大鼠EGFR、FABP、Glu、Asp表达显著高于对照组(P<0.05)。模型组大鼠血管壁厚度显著高于对照组(P<0.05)。EGFR、FABP和Glu、Asp对蛛网膜下腔出血诊断的敏感度、特异度、准确度存在差异,Glu、Asp单独检测敏感度显著高于其他两项,差异有统计学意义(P<0.05);四项联合检测敏感度、特异度、准确度显著高于单项检测,差异有统计学意义(P<0.05)。结论 EGFR、FABP、Glu、Asp在蛛网膜下腔出血大鼠的血清中表达水平较高,在蛛网膜下腔出血的诊断中起到非常重要的辅助作用,且四项指标联合检测的敏感度、特异度、准确度较高,能够提升诊断效能,对蛛网膜下腔出血的治疗与预后提供重要的参考价值。
        Objective To investigate the expression levels and significance of epidermal growth factor receptor(EGFR),fatty acid binding protein(FABP), glutamate and aspartate in serum of rats with subarachnoid hemorrhage.Methods Forty healthy male SD rats were randomly divided into control group and model group,with 20 rats in each group. The rat models with subarachnoid hemorrhage were established.The serum levels of EGFR, FABP, Glu and Asp were detected by HPLC. The expression levels of EGFR, FABP, Glu and Asp were detected by Western Blot. The cross-sectional area and vascular wall thickness of basilar artery of rats were measured under optical microscope. Moreover the specificity, sensitivity and accuracy of EGFR, FABP, Glu and Asp by simple index detection and combination detection of the four indexes were analyzed and compared.Results The serum levels of EGFR, FABP, Glu and Asp in model group were significantly higher than those in control group(P<0.05). The expression levels of EGFR, FABP, Glu and Asp in model group were significantly higher than those in control group(P<0.05). The vascular wall thickness in model group was significantly greater than that in control group(P<0.05).Moreover there were significant differences in the specificity, sensitivity and accuracy of EGFR, FABP, Glu and Asp by simple index detection and combination detection of the four indexes in diagnosis of subarachnoid hemorrhage in rats.The sensitivity of simple Glu detecton or simple Asp detection was significantly higher than that by simple detection of the other two indexes(P<0.05). The sensitivity, specificity and accuracy of combination detection of the four indexes were significantly higher than those by simple detection of any simple index detection(P<0.05). Conclusion The serum levels of EGFR, FABP, Glu and Asp in rats with subarachnoid hemorrhage are hgher, which plays an important role in the diagnosis of subarachnoid hemorrhage,moreover, the sensitivity, specificity and accuracy of combination detection of the four indexes are higher,which can improve the diagnostic efficiency,which provides important references for the treatment and prognosis evaluation of subarachnoid hemorrhage.
