胃癌中微小RNA-379表达及与临床病理特征的关系

摘要
目的 探讨微小RNA(miR-379)在早期胃癌、癌前病变患者组织和血浆中的表达差异,及其表达与早期胃癌临床病理特征的关系。方法 收集2016年1月至2017年12月在我院就诊的113例胃癌患者癌组织和47例癌前病变组织,采集所有受试者的血液样本,提取组织及血液总RNA,实时荧光定量PCR(QPCR)检测miR-379表达水平。采用Spearman相关性分析组织和血浆中miR-379表达水平的关系。绘制受试者工作特征(ROC)曲线评估miR-379表达对早期胃癌的诊断价值。分析胃癌组织和血浆中miR-379表达与早期胃癌患者临床病理特征的关系。结果 早期胃癌组织和血浆中miR-379表达水平分别为0.57±0.28和0.28±0.15,低于癌前病变组患者组织和血浆中的0.86±0.35和0.39±0.18,差异有统计学意义(P<0.05)。Spearman相关性分析结果显示,113例胃癌患者组织和血浆中miR-379表达水平呈正相关(r=0.846,P<0.05)。经ROC曲线分析,组织和血浆miR-379表达对早期胃癌诊断的ROC曲线的AUC分别为:0.735(95%CI:0.653~0.816)、0.684(95%CI:0.599~0.768),cut-off值分别为0.59和0.24。根据cut-off值,66例患者组织miR-379呈低表达,47例患者组织miR-379呈高表达; 56例患者血浆miR-379呈低表达,57例患者血浆miR-379呈高表达。组织miR-379表达与肿瘤直径、浸润深度和分化程度有关(P<0.05);血浆miR-379表达除了与肿瘤直径、浸润深度和分化程度有关外,还与淋巴结转移和淋巴管浸润有关(P<0.05)。结论 早期胃癌组织和血浆中miR-379表达下调,且与不良临床病理特征有关,血浆miR-379表达有望成为早期胃癌诊断和评估恶性程度的特异性分子标志物。
        Objective To investigate the expression of microRNA-379(miR-379) in tissues and plasma of patients with early gastric cancer and precancerous lesions, and its relationship with clinicopathological features of early gastric cancer. Methods From January 2016 to December 2017, 113 cases of gastric cancer and 47 cases of precancerous lesions were collected. Blood samples were collected from all subjects. Total RNA was extracted from tissues and blood. The expression of miR-379 was detected by real-time fluorescence quantitative PCR(QPCR). Spearman correlation analysis was used to analyze the correlation between the expression of miR-379 in tissues and plasma. The ROC curve was drawn to evaluate the diagnostic value of the expression of miR-379 in early gastric cancer. The relationship between the expression of miR-379 in gastric cancer tissues and plasma and the clinicopathological characteristics of early gastric cancer patients was analyzed. Results The expression levels of miR-379 in tissues and plasma of early gastric cancer were 0.57±0.28 and 0.28±0.15, respectively, lower than those in tissues and plasma of patients with precancerous lesions(0.86±0.35, 0.39±0.18), with statistical significance(P<0.05). Spearman correlation analysis showed that the expression of miR-379 in tissues and plasma of 160 patients was positively correlated(r=0.876, P<0.05). According to ROC curve analysis, the AUC of the ROC curve of tissue and plasma miR-379 expression for early gastric cancer diagnosis was 0.735(95%CI:0.653-0.816),0.684(95%CI:0.599-0.768), and the cut-off values were 0.59 and 0.24, respectively. According to cut-off value, the expression of tissues miR-379 was low in 66 patients and high in 47 patients; the expression of plasma miR-379 was low in 56 patients and high in 57 patients. The expression of tissue miR-379 was correlated with tumor diameter, depth of invasion and degree of tissue differentiation(P<0.05). The expression of plasma miR-379 was related not only to tumor diameter, depth of invasion and degree of tissue differentiation, but also to lymph node metastasis and lymphatic vessel infiltration(P<0.05). Conclusions The expression of miR-379 in early gastric cancer tissues and plasma is down-regulated, which is related to the poor clinicopathological characteristics. The expression of miR-379 in plasma is expected to become a specific molecular marker for early gastric cancer diagnosis and evaluation of malignancy.
