超微血管成像技术在判定浅表淋巴结良恶性中的应用

摘要
目的探讨超微血管成像技术在判定浅表淋巴结良、恶性中的应用。方法对150个浅表淋巴结的超声数据进行回顾性分析,并与病理结果进行比较。比较二维灰阶超声联合CDFI和SMI诊断淋巴结病变的准确性,并利用SMI技术分析淋巴结内部的血流分布类型。结果 150个淋巴结中,反应性淋巴结66个,转移性淋巴结59个,淋巴瘤25个,二维灰阶超声联合SMI技术诊断的准确率为84.9%,二维灰阶超声联合CDFI诊断的准确率为71.1%,差异有统计学意义(P<0.05)。采用SMI技术,良性淋巴结内以无血流型及中央型为主(83.1%),转移性淋巴结以周边型及混合型为主(81.7%),淋巴瘤以中央型为主(52.0%)。结论 SMI比较CDFI更能敏感的检测出淋巴结内部及周边的血流信息,为临床确定淋巴结性质提供更加全面、准确的信息。
        Objective To investigate the application of ultra-microvascular imaging in the determination of benign and malignant superficial lymph nodes. Methods The ultrasound data of 150 superficial lymph nodes were retrospectively analyzed and the results were compared with pathological results. The accuracy of two-dimensional gray-scale ultrasound combined with CDFI and SMI in the diagnosis of lymph node lesions was compared, and the type of blood flow distribution within the lymph nodes using SMI technique was analyzed. Results Among the 150 lymph nodes, 66 were reactive lymph nodes, 59 were metastatic lymph nodes, and 25 were lymphoma. The accuracy of two-dimensional grayscale ultrasound combined with SMI technique was 84.9%, and the accuracy of two-dimensional gray-scale ultrasound combined with CDFI was 71.1%. The difference was statistically significant(P<0.05). SMI technique was used. Benign lymph nodes are mainly composed of no blood flow type and central type(83.1%). Metastatic lymph nodes were mainly peripheral type and mixed type(81.7%), and lymphomas were mainly central type(52.0%). Conclusion Compared with CDFI, SMI is more sensitive to detect blood flow information inside and around lymph nodes, providing more comprehensive and accurate information for clinically determining lymph node properties.
