摘要
目的:为了提高对真菌性外耳道炎的认识,并找到一种合理的治疗方法,减少其复发。方法:对167例患者耳内分泌物行细菌及真菌培养,选用两种常用的抗真菌药物派瑞松及达克宁进行治疗,每一组内再分成患者自行涂抹药物组及耳内镜下外耳道给药组,分别比较两种药物及两种给药方式的有效率。结果:培养出191株致病真菌中,曲霉菌145例,酵母菌38例,念珠菌8例;共有24例患者同时培养出致病细菌;内镜下给药组有效率明显高于患者自行涂抹药物组,派瑞松组有效率高于达克宁组。结论:真菌性外耳道炎主要的致病真菌是曲霉菌和酵母菌,耳内镜下外耳道填塞派瑞松是治疗真菌性外耳道炎简单、有效、依从性好的方法。
Objective: To improve the understanding of otitis external mycotica, and find a more reasonable treatment and reduce its recurrence. Methods: 167 patients of otitis external mycotica had the bacterial and fungal cultures,and treated with two kinds of commonly used antifungal agents, Pevisone and Dekening. The patients were treated by two different mode of administration in each group: Patient self-coated drugs and Endoscopic administration by doctor, and then to compare the efficacy of the two drugs and the two modes of administration. Results: Among the 191 pathogenic fungi, 145 cases of Aspergillus, 38 cases of Yeast and 8 cases of Candida were isolated. 24 patients were cultured with pathogenic bacteria. The effective rate of Endoscopic administration was higher than Patient self-coated drugs, and Pevisone was more effective than Dekening. Conclusion: The main pathogenic fungi of otitis external mycotica is Aspergillus and Yeast, and Endoscopic administration with Pevisone is a simple, effective and reliable method for the treatment of otitis external mycotica.
